Clinical treatment of pulp necrosis in wedge-shaped defects is mainly root canal treatment followed by filling treatment or crown restoration. Wedge defects are chronic, non-carious injuries to the neck of the tooth. If the wedge-shaped defect is too deep, it can involve the pulp and cause pulpal necrosis. This should be treated accordingly, mainly with root canal therapy to remove the necrotic pulp tissue. Afterwards, the wedge-shaped defect can be filled. If the defect is so deep that it has or almost has caused a transverse fracture of the tooth, it should be restored with a crown, or if necessary, a crown with a core.
