Double decompression of the hip joint with Chinese herbal medicine for early ischemic necrosis of the femoral head

  Patient Liu, male, 41 years old, presented with right hip pain for more than one month without any obvious reason, aggravated for one week. The X-ray and MR film showed that the right femoral head was ischemic necrosis and the right Ficat stage II. The patient used to drink alcohol, about seven taels a day, and came to our hospital for treatment.  On admission, he had a walking limp that was progressively worsening, pressure pain at the midpoint of the right groin (+), pain on movement of the right hip joint, 0° extension, 90° flexion, 15° internal rotation, 25° adduction, abduction and external rotation were basically normal, “4” test (+), Allis sign (-), mobility of the right knee and ankle joint The peripheral blood circulation, sensation and movement of the affected limb were normal. After admission, he underwent femoral head decompression and intra-articular double decompression, and after surgery, he was instructed to perform functional exercises with the help of double crutches, while he was given a femoral head necrosis healing capsule orally. Six months after the operation, the patient abandoned the crutches and walked around. 2.5 years later, the gait was normal and the right hip joint was only slightly painful after long distance walking. The right hip joint had 0° of extension, 110° of flexion, 10° of posterior extension, 20° of internal rotation, and basically normal external rotation, internal retraction and abduction activities.
