How much do you know about femoral head necrosis?

  Femoral head necrosis is a common disabling disease of the hip joint in China. It is caused by impaired circulation in the bone and death of bone cells, which subsequently leads to structural changes in the femoral head and later femoral head collapse, hip joint pain and functional impairment. There are many causes of femoral head necrosis, which are generally divided into two categories: traumatic and non-traumatic.  First, traumatic femoral head necrosis Trauma causes femoral head necrosis, such as femoral neck fracture caused by external impact, hip dislocation, hip trauma, hip joint sprain and contusion, etc. It can directly or indirectly damage the blood flow of the femoral head, thus leading to ischemic necrosis of the femoral head.  Trauma is the main factor causing femoral head necrosis. However, the occurrence and extent of traumatic ischemic necrosis of femoral head mainly depends on the degree of vascular destruction and the compensatory ability of collateral circulation.  Second, non-traumatic femoral head necrosis Non-traumatic femoral head necrosis has many causative factors, including alcoholism, after high-dose hormone therapy, after local radiation therapy, decompression sickness, systemic lupus erythematosus, coagulation dysfunction, hemoglobinopathy, and so on. Among them, alcoholism and hormones causing femoral head necrosis are the most common causes in China.  Drugs cause femoral head necrosis, such as long-term use of hormonal drugs due to bronchitis, asthma, rheumatism, rheumatoid, neck, shoulder, back and leg pain, diabetes, skin disorders, etc. It is an early statement that the accumulation of hormones in the organism due to the large amount or long-term use of hormones has led to the development of the disease.  Recently, it is believed that the occurrence of femoral head necrosis is directly related to the type, dosage form and route of administration of hormones, and is not directly proportional to the total amount and duration of hormones. However, the long-term use of large amounts of hormones or excessive daily doses and sudden changes in dose increases and decreases are also a cause of femoral head necrosis.  Alcohol stimulation causes femoral head necrosis. The accumulation of alcohol in the body due to long-term heavy drinking leads to the increase of blood lipids and the damage of liver function. The increase in blood lipids causes an increase in blood viscosity, slows blood flow, and changes blood coagulation, which can lead to blockage of blood vessels, bleeding or fat embolism, resulting in osteonecrosis. Clinical manifestations include aggravation after drinking, walking duck walk, heart failure, weakness, abdominal pain, nausea and vomiting.  Non-traumatic osteonecrosis of the femoral head occurs in 20-50 years of age, is more common in men, and accumulates in both hips in about 80% of patients. According to the study of the natural course of the disease, without limited treatment, most patients experience femoral head collapse within 1-4 years. Once the femoral head collapses, approximately 87% of patients require total artificial hip replacement due to hip dysfunction.  After the related factors lead to impaired circulation within the bone, tissue ischemia and hypoxia, and bone tissue necrosis, which then leads to structural changes in the femoral head, causing femoral head collapse, hip pain and dysfunction. The disease starts insidiously and lacks specific manifestations at the beginning, which is often ignored by patients. The disease develops quickly, and when the symptoms worsen and the patient cannot tolerate it, it has often progressed to advanced stage.
