Minimally invasive treatment of hemorrhoids

The TST procedure follows the formation mechanism of hemorrhoids in human body and is designed according to the physiopathology of hemorrhoids. It is designed to correct the pathophysiological changes of hemorrhoids instead of removing the entire anal cushion, preserving the normal anal cushion and mucosal bridge, maintaining the fine function of the anus, reducing surgical trauma, shortening treatment time, and making hemorrhoid surgery more minimally invasive. The doctrine of inferior displacement of the anal cushion was proposed by Thomson in 1975, which believes that the anal cushion is a normal tissue composed of mucosa, blood vessels and fibrous support structures in the inner wall of the anal canal, which has an irreplaceable role in maintaining the self-control function of the anus, and can only be called “hemorrhoid” or “hemorrhoid” when it has pathological changes or abnormal displacement and clinical symptoms appear. “Hemorrhoid disease”. In 1998, Longo first reported the use of PPH surgery to treat hemorrhoids based on the theory of anal cushion subluxation, and it has been widely used in clinical practice since then. Since the introduction of PPH, we have gradually realized its shortcomings while achieving better results. As PPH surgery adopts the mucosal circumcision on hemorrhoids, the prolapsed mucosa is removed along with the less symptomatic or even normal mucosal tissue, which causes some unnecessary damage to the anus and may increase postoperative anal pain and cause complications such as anal stenosis. The TST surgery, based on the theoretical basis of PPH surgery “suspension”, “cutting off the flow” and “reducing the accumulation”, adopts the method of correcting only the pathophysiological structure changes of hemorrhoid lesions, preserving the normal mucosal tissue and the normal mucosal bridge, thus reducing the occurrence of postoperative complications and effectively preventing stenosis. At the same time, the number of implanted titanium nails is reduced, which can reduce the discomfort of the anus and maintain the fine function of the anus, which is more in line with the contemporary concept of minimally invasive hemorrhoid surgery than PPH surgery. Based on the theoretical basis of “suspension”, “disconnection” and “reduction of accumulation” of PPH surgery, the operation adopts the method of correcting only the pathophysiological structural changes of hemorrhoid lesions.
