Clinical data: 18 of 28 cases were circumcised and 10 were circumcised. The age ranged from 4 months to 12 years, with an average of 7 years. The plastic ring sleeve was made of hard plastic and was divided into two parts: the ring body and the ring handle. The ring body resembled an ancient bell shape, and the diameters were 13, 15, and 17 mm, respectively. The dorsal penile nerve block was anesthetized with 1% lidocaine, and some older children were anesthetized under the skin on the ventral side of the penis. The foreskin is retracted to the posterior coronary sulcus (in circumcised children, the external foreskin opening is strongly dilated with vascular forceps to allow for externalization and separation of adhesions) and the inner foreskin plate and coronary sulcus are removed and disinfected again. The foreskin is pressed with a vascular forceps at the dorsal median cut of the foreskin, the foreskin is cut along the pressed area, and a foreskin ring matching the size of the glans is placed between the inner plate of the foreskin and the head of the penis, and the distal foreskin is tied with a “4” wire so that the distal end is ischemically pale. The excess foreskin at the distal end of the ligature was cut off, the ring stem was broken, and iodophor water was applied to the wound. In this group, the operation time was 2~5 min, and the bleeding was 0~1 mL. There was no postoperative complications such as bleeding, infection, slipping of the ligature, shortening of the tether, narrowing of the foreskin, etc. One case had pain at the foreskin ligature 6 h after surgery, and the pain disappeared after intramuscular injection of prednisolone, and one case had foreskin edema 1 d after surgery without any treatment, and the edema subsided 2 d later. Discussion: Circumcision using plastic ring sleeve has been carried out for many years in foreign countries, because it has the advantages of simple and easy operation, short time, neat edge and convenient postoperative care, etc. In recent years, many hospitals in China have carried out this procedure, especially for the treatment of pediatric prepuce and circumcision. For the plastic ring circumcision of adults, there are also related reports in China. Since the foreskin of adults is thicker, the ligature line is not tight, and the penis is prone to erection after surgery so that the ligature line slips and bleeds, so it is not yet carried out in our hospital. Operation precautions: 1, the dorsal foreskin should not be cut too long, otherwise the ligation may make part of the foreskin cut edge leak, causing postoperative bleeding. 2.The size of the ring sleeve is appropriate to avoid compression of the urethral orifice by the ring sleeve, which may cause dyspareunia. 3, retain the ties can not be too short, ligature line as far as possible in the ring sleeve groove. 4, after ligation cut excess foreskin, the residual length of 2mm is appropriate, too little easy to host back, causing bleeding; too much foreskin sleeve is not easy to fall off.
