Analyze the causes of wrinkles to choose facial wrinkle removal methods

The main factors for wrinkle formation are the degeneration of skin elastic tissue and the contraction of repeated facial and neck muscle tissue. When you are young, the face may only have dynamic wrinkles with facial expressions, for example, frown lines between the eyebrows, crow’s feet at the corners of the eyes when smiling, forehead lines when raising the eyebrows, and other expression lines, while there are no wrinkles on the face in a calm state. As we get older, static wrinkles will gradually appear, and even the creases on the outside of the upper eyelid will appear due to the drooping of the eyelid, resulting in a “triangular eye” appearance. Depending on the depth of expression wrinkles and the degree of skin redundancy, expression wrinkles, furrows and folds can be classified from shallow to deep, and this classification is meaningful from a treatment perspective. For expression wrinkles and folds, there are many comprehensive treatment options, including filler injections, botulinum toxin injections, selective muscle excision, and laser resurfacing. For the correction of skin folds, surgical tightening is required (e.g., upper eyelid lift, eyebag removal, facial wrinkle removal, etc.). However, according to the cause of wrinkles, the only way to fundamentally remove unpleasant expression lines is to remove the driving force that creates them, so selective treatment of some areas of the expression muscles so that they lose their function of creating wrinkles becomes the root of the treatment. The use of Botox can paralyze the local expression muscles to achieve the function of wrinkle removal, but after injection, there are disadvantages such as excessive dullness of local expression, short duration of action (4~6 months), and local sagging (for example, after Botox injection in the forehead, although the forehead lines are removed, it aggravates the sagging of the eyebrows and upper eyelids). In contrast, a highly selective treatment of the expression muscles (partial frontalis – head-lift lines; frown muscles – frown lines; partial orbicularis oculi – crow’s feet at the corners of the eyes) using a minimally invasive surgical method with endoscopic technology can achieve the desired results: natural expression, once and for all, and simultaneous lifting of the face. In contrast, the effect of injectable fillers is very limited, while skin lasers can only improve the extremely fine wrinkles of the facial skin in the short term.
