Head malformation includes large head, small head, double head, etc.; neck malformation includes thick neck, fracture, etc.; tail malformation includes coiled tail, double tail, broken tail, etc. According to the WHO Human Semen Examination and Laboratory Manual (5th edition) testing standards, sperm malformation rate >96% is considered malformed spermatozoa. In other words, the sperm malformation rate of 90% mentioned by the patient above is completely normal and there is no need to worry. So why is the sperm malformation rate so high? This is because sperm are affected by many factors in the process of generation as well as maturation, and the head, neck and tail may all have abnormal morphology, plus the fifth edition of testing standards are more stringent, so the sperm malformation rate will be higher. Therefore, sperm malformation is a normal phenomenon and there is no need to worry as long as it is within the normal range. So why is it that some patients were previously tested in other hospitals with a sperm malformation rate of only 20%? There may be several reasons for this: first, the testers are not professional technicians in semen testing, or have not yet undergone systematic training, and are not familiar with sperm morphology testing methods and testing standards, and lack experience; second, the testers do not understand the fifth edition of sperm morphology testing methods, or still use the previous testing standards. For sperm morphology testing, the sperm should first be stained and stained, and then carefully observed under a microscope, and generally at least 200 sperm should be tested. After staining, a sperm malformation rate of ≤96% (i.e., the proportion of normal morphology sperm ≥4%) falls within the normal range. A spermatozoa percentage of normal form <4% is considered as teratozoospermia. So, how should teratozoospermia be treated?
