The melanocytes exist in the basal layer of the epidermis, and the abnormality or absence of melanocytes leads to the formation of skin white spots. Pre-operative evaluation: The whole body has no enlarged or new white spots in the past 6 months as judged by the physician, which means it is in the stable stage; the white spot area is <250px2, and white spots larger than this area may need to be divided into multiple batches; there is no local scar or infection in the white spot; the white spot area is flat and easy to fix. Hair and joint areas are not recommended for epidermal grafting because of the difficulty of fixation with pressure bandaging and low survival rate of skin fragments. Epidermal negative pressure herpetografting steps: 1. Select a suitable donor area (such as abdomen, inner thighs, etc.), select the appropriate hole for the herpetological head according to the area of the white spots, disinfect the epidermis with the epidermis separator after negative pressure to suck the epidermis, the time is generally 30~50 minutes, see the full blister when the sucking is successful, the wall of the blister is the separated epidermis; 2. After the anesthesia is completed, the skin area is treated with a high-speed abrasive tip; 4. After the epidermal separation is completed, the herpes wall is removed with ophthalmic scissors and ophthalmic forceps, and the skin pieces are carefully organized to make them flat and transplanted to the ground skin area in a timely manner; 5. After 7 days of follow-up, the bandage will be opened and normal life can be resumed, at this time the white spots can still be attached to part of the herpetic wall; 7. It takes 3-6 months to recover for large areas. The recovery time of white spots in uncovered areas between skin patches is about 3 months later than that of covered areas. Some patients may experience darker than normal skin color in the original white spot area, which can gradually return to normal in a few months to a year or so; 8. If you want to accelerate the re-coloration, in addition to oral and topical medications, you can cooperate with external treatments such as 308 excimer light or fire needling, especially for the interstitial areas not completely covered by the skin flakes.
