The effect of obesity on sexual function

As people’s living standards continue to improve, more and more people are becoming obese, and at younger and younger ages. How to determine whether a person is considered fat? In other words, what is the standard of obesity? At present, the international common use of “body mass index” (BMI) as the standard for determining obesity, according to the World Health Organization: body mass index = weight (kg) ÷ height (m) squared, normal 18 ~ 23, more than 24 for overweight, more than 28 for obese. For example, a middle-aged male, height 1 m 70, if the weight of 60kg, 60 divided by 1.7 squared, the body mass index is equal to 20.76, normal weight; if 70kg, 70 divided by 1.7 squared, equal to 24.22, just over 24, is a little overweight; if the weight of 85kg, the calculation is 29.41, more than 28, that is obese. For a long time, some people always think that as long as the growth of round waist body fat, it means good nutrition. In fact, obesity is not equal to good nutrition, it is the same as wasting, are nutritional disorders caused by, the two are different is only the body fat storage of how much. Obesity is not only an excess of body fat, but also the presence of certain nutrients lack. Such as many obese children, the body is often accompanied by iron, calcium and other micronutrient deficiencies, causing iron deficiency anemia, chondromalacia and other nutritional deficiency disorders. Obesity is a disease, due to energy intake more than consumption, resulting in excessive fat accumulation in the body, in addition to excessive accumulation of subcutaneous fat, viscera and throughout the body’s blood vessels are also full of fat, so it is easy to cause a variety of vascular disease, especially the health and life of serious cardiovascular disease. Relevant research data show that the incidence of hypertension in obese people is 1.5 to 3 times that of normal people. Moreover, the higher the degree of obesity, the higher the incidence of hypertension, and the more obvious the increase in blood pressure. The incidence of hypertension in severely obese people is as high as 50% or more. In addition to cardiovascular and cerebrovascular diseases, obesity also tends to induce diabetes. Although it can not be said that obesity is the direct cause of diabetes, but it is not negligible to the role of diabetes induced. Many data confirm that the heavier the degree of obesity, the higher the incidence of diabetes. At present in some economically developed countries, obesity caused by the disease, the most is diabetes. About one-third of the adult type diabetes patients belong to the obese body type. Obesity is prone to exercise system disorders. The excessive weight gain of obese people is an extra burden on the motor system such as bones and joints, especially on the spine and lower limbs. Bones, joints and other organizations for a long time to support the excessive weight, as if carrying extra things every day, over time, is bound to accumulate labor into disease, arthritis, muscle strain or spinal nerve root compression, causing waist, leg, shoulder and back pain, and even cause joint deformation, seriously affect limb activities. Obesity is easy to cause a variety of other diseases, like gallstones, fatty liver, cirrhosis of the liver, etc. Obese people are prone to eczema, psoriasis (psoriasis), hair keratosis, seborrheic dermatitis, acanthosis nigricans and many other skin diseases. So, obesity has become a serious health problem. But do you know? Obesity and sexual health are also closely related. Many people who are too obese have some problems with sexual function to varying degrees. An Indian sexologist recently published a report on the relationship between sexual function and obesity. He pointed out that for every 5 kg increase in body weight of men, their genitals will be “shortened” by 1 cm. The reason for the “shortening” is that the external genitalia are buried in thick fat. He introduced a case: a 1.70-meter-tall man, weighing 100 kg, obese, he basically did not have sex, because the penis has “shrunk” to the abdominal fat, only a small part of the genitals can be seen vaguely. In obese men, some are accompanied by hypogonadism. Some men are slightly fat and have normal sexual function, while about half of those with obesity have low sexual function. The reasons for reduced sexual function in obesity may be: ① excessive obesity, blood lipids also increase, affecting various metabolic functions of the liver, reducing the ability to inactivate estrogen, enhancing the conversion of androgens into estrogen, increasing the level of estrogen in the blood and reducing the secretion of androgens, which in turn leads to decreased libido and erectile dysfunction (impotence) and low sexual function. As we all know, male sexual function is positively correlated with androgens in the blood. If the level of androgens in men’s body is low, sexual function will be weakened to different degrees, mainly in terms of libido, erection, ejaculation during sex and the feeling of orgasm, which are obviously inferior to the normal weight period. ② Obese people are prone to combined cardiovascular and metabolic diseases, these diseases themselves can affect the male sexual function to some extent. In addition, the body fat person, the amount of activity is small, easy fatigue, sometimes premature ejaculation, may be one of the other factors for the reduction of sexual function. Obese people because of their size, will increase the difficulty of sexual intercourse, such as clumsy, single position, etc.. If there is often a failure of sexual intercourse, will inevitably lose interest in sex, resulting in sexual apathy. In addition, obese people due to the reason of body shape, in social, love, marriage and other aspects are not very smooth, easy to lose confidence, doubts about their own sexual ability, resulting in psychological pressure, thus causing lower libido and other sexual dysfunction. A 2008 paper in the Journal of Practical Medicine, “Investigation and analysis of the relationship between obesity and sexual dysfunction in white-collar men,” investigated 800 cases of married men 800 cases, with occupations dominated by white-collar brain workers, including 250 cases of teachers, 190 cases of doctors. There were 120 cases of organ workers and 240 cases of white-collar workers in companies. The age ranged from 26 to 46 years old, with an average of 37 years old. The group was divided into two groups: 348 cases in the overweight group and 452 cases in the obese group. The questionnaire was designed in tabular format, including: (1) male sexual desire, the number of erections per week, the number of completed sexual intercourse per week, and the number of successful ejaculation per week (male sexual desire, erection, sexual intercourse, ejaculation and other sexual activity links on any one of the obvious functional reduction or disorder), the results: for sexual desire disorder, 54 cases (16%) in the overweight group, 136 cases (30%) in the obese group; erection disorder, 98 cases (28%) in the over 98 cases (28%) in the overweight group, 232 cases (51%) in the obese group; sexual intercourse disorder, 128 cases (37%) in the overweight group, 316 cases (70%) in the obese group; ejaculation disorder, 139 cases (40%) in the overweight group, 347 cases (77%) in the obese group, the difference in sexual dysfunction caused by the two groups with different degrees of obesity was significant, and the sexual dysfunction of obesity was significantly more than that of overweight. It means that the more serious the degree of obesity, the more frequent and serious the sexual dysfunction caused. In addition, many obese men will also have a phenomenon, the old people called ‘burning crotch’, that is, because the inner thighs are too fat, skin tightly next to, repeatedly rubbing, resulting in the testicles are always in a higher temperature environment, making fertility decline. Why do you say that high testicular temperature will affect fertility? It turns out that the formation of sperm in the testes, the temperature conditions needed to be 3 ℃ ~ 5 ℃ lower than the body temperature, if the temperature is too high, the same as the body is 36 ℃ ~ 37 ℃, then, sperm production will be seriously affected, so the testes must protrude out of the body, and the scrotum is the heat regulating organs. And again, women. Normal women have more subcutaneous fat than men, especially in the breast, abdomen, buttocks and thighs and other areas are more obvious, which also forms the female body beauty. But abnormal obesity not only makes women lose their physical beauty, but also can bring some troubles, such as metabolic disorders, menstrual disorders, and some of them can also occur infertility. What causes this kind of morbid obesity? Some endocrine diseases (hypothalamus, pituitary gland, thyroid, gonads, adrenal glands), central nervous system diseases, pancreatic diseases, etc. can cause obesity, these are called symptomatic obesity, mostly caused by organic lesions. Another kind of obesity is called somatic obesity, also called simple obesity, the main reason is over-eating, plus some long-term sitting work, too little activity, excess energy in the form of fat accumulation, the vast majority of obese women belong to this simple obesity. Physical obesity is the most important cause of infertility in obese women, often occurring during puberty, after childbirth and other major changes in the endocrine status of the body. The incidence of menstrual disorders (mostly scanty menstruation) in obese women can reach 56.2%, while only 38.8% in lean women; the basal body temperature monophasic in obese women is 52.1%, while in lean women is 31.8%. So, why do menstrual disorders and infertility occur in obese women? Most of these changes are related to changes in hormone levels. The cause of obesity is often the cause of infertility. These obese women have reduced pituitary gonadotropin secretion, hypogonadism, and amenorrhea, while menstruation can often resume when weight loss occurs. Obesity reproductive incompetence dystrophy syndrome, caused by hypothalamic lesions. Since obesity affects sexual function so much, it is important to mention weight loss. Maintain a good shape, the pursuit of beauty is good, but everything has a degree, have done the head will affect health, some girls because of excessive weight loss is suffering from anorexia nervosa. A high school girl, because someone said she was plump and began to blindly lose weight, not after a few months weight is down, but people are too weak to even go to gym class, and also menstrual disorders, and finally even amenorrhea. Over the years, the incidence of anorexia nervosa has increased significantly, especially among adolescent girls. In fact, it was observed more than 20 years ago that patients with anorexia nervosa have an aversion to sex, they know very little about the anatomy of female reproductive organs, menstrual physiology, fertility and contraception, sexual knowledge, they never read books related to reproduction and sex, they do not like their gradually developing breasts, they are averse to menstruation, they are averse to genitalia, they are reluctant to get married when they reach marriage age, they have a low sex drive after marriage, they are not interested in Their own sexual feelings and sexual behavior often feel guilty and self-condemnation, pain or discomfort during sexual intercourse, can not enjoy sexual pleasure, immature marital relations, some women also have irregular menstruation or even amenorrhea. So it is important to lose weight scientifically.
