Conservative treatment of femoral head necrosis

  Which femoral head necrosis is suitable for conservative treatment?  Conservative treatment of femoral head necrosis is still in the exploratory stage and its efficacy is influenced by many factors, early diagnosis and treatment are especially important. Conservative treatment is suitable for X-rays, magnetic resonance imaging (MRI) and other examinations, which have not yet collapsed, and confirmed by the specialist that there is a possibility that collapse will not occur. That is, patients with stage I or II femoral head necrosis, and the necrosis is small in extent, and the necrotic area is deviated below the weight-bearing area or located in the center of the femoral head.  Or if the general condition is not suitable for surgery, such as serious medical disease, conservative treatment can be applied to relieve symptoms and preserve hip function as much as possible. Otherwise, conservative treatment is not appropriate. Avoid weight-bearing: Strictly limit weight-bearing and require patients to hold double crutches or even sit in wheelchairs, and patients must cooperate. The purpose is to protect the ischemic and necrotic femoral head from pressure and provide an opportunity for ischemic tissue repair to control the development of lesions, prevent collapse and facilitate the repair of the ischemic and necrotic femoral head. However, it is generally believed that restricting weight-bearing cannot completely save the development of femoral head necrosis. This method is mainly applied to elderly patients who are not suitable for surgical treatment, poor general condition, progressive ischemic necrosis and patients with poor prognosis. The possibility of healing and lesion stabilization is related to the size of the lesion and the distance from the joint surface: if the lesion is small or far from the joint surface, it can mostly heal or stabilize; if the lesion is adjacent to the joint surface or the lesion is large in scope, the possibility of stabilization is extremely small even without weight bearing.  2, drug treatment: for fat metabolism disorders and intravascular coagulation and other etiological theory, the application of lipid-lowering drugs and anticoagulants for the treatment of hormonal femoral head necrosis, providing a new idea for drug prevention and early treatment. Studies have confirmed that high-dose application of hormones along with earth kinase and aspirin can slow down the process of hormonal osteonecrosis and have a role in preventing hormonal osteonecrosis. The application of western drugs for the treatment of femoral head necrosis is less reported, and in short, the effect of drug treatment is not yet certain. Chinese herbal medicine is the advantage of our country, and it has been initially shown to have unique efficacy on the disease.  3.Electrical stimulation: it has osteogenic effect and can promote fracture healing. Electrical stimulation can be used as an independent treatment method for osteonecrosis or as an adjunct to surgery.  4, other treatment methods: hyperbaric oxygen therapy, etc., not many reports, the effect to be further determined.  Femoral head necrosis repair is very slow, conservative treatment course usually need to last 3-5 years, or even longer, during the treatment period need to take medication as required by the doctor, do not intermittent. It is important to follow up with the specialist regularly, take X-rays and MRI if necessary, so as to understand the changes of the disease and adjust the treatment accordingly.
