Ulcerative colitis is a chronic non-specific inflammatory bowel disease with abdominal pain, diarrhea, mucus or pus and blood stools as the main clinical manifestations, which is classified as “diarrhea” in Chinese medicine.
At present, the treatment of this disease with western medicine has many adverse reactions and high recurrence rate. The recent healing rate of ulcerative colitis treated with interdigitated moxibustion is 48.57%, therefore, the clinical efficacy and mechanism of ulcerative colitis is of great significance.
I. Introduction of the therapy
This therapy is a distinctive Chinese medicine treatment with distinctive Chinese medicine characteristics established through long-term clinical research under the guidance of Chinese medicine theory. After multi-center and large sample study, it is proved that the clinical healing rate of interdigitated moxibustion therapy is 48.81% and the total effective rate is 91.67%. The study proves that this therapy has certain efficacy advantages over western medicine treatment, and this method is safe, reliable, easy to operate, economical and practical.
Diagnostic criteria
Referring to the diagnostic criteria of the Chinese and Western medicine classification of ulcerative colitis of the Chinese Society of Integrative Medicine and Digestive Diseases in 1992, patients with ulcerative colitis mainly of the weak spleen and stomach type were selected.
Main symptoms.
①Diarrhea with loose stools, mucus and a small amount of pus and blood in the feces;
②Low food and poor appetite;
③ abdominal distension after eating;
④Tongue is pale, fat or with teeth marks, with thin white coating;
(⑤) Weak or slow pulse.
Secondary symptoms.
① abdominal distension and intestinal tinnitus;
② abdominal pain and pressure;
③Lethargy of the body;
④Fatigue and laziness in speech;
⑤ yellowish complexion;
(6) Edema of the intestinal mucosa is more obvious than congestion;
(7) superficial intestinal mucosal ulcers, the surrounding redness and swelling is not obvious, the surface is white secretion;
(8) Rough and granular intestinal mucosa.
The evidence type can be determined by having 2 main symptoms and 2 secondary symptoms, or 1 main symptom and 3 secondary symptoms.
Contraindications
1.Patients with severe ulcerative colitis;
2, combined with the cardiovascular system, liver, kidney and hematopoietic system and other serious life-threatening primary diseases and psychiatric patients;
3.Patients with local skin erosion, ulcers, carbuncles and other infections and skin diseases;
4.Patients who are not identified by Chinese medicine as having this type of disease;
5.Patients who are pregnant or lactating, etc.
Operation method
1.Apparatus preparation
Medicine cake moxibustion composition: 10 grams of Radix Aconiti, 2 grams of cinnamon, 3 grams of Danshen, 3 grams of safflower, 2 grams of moxa.
Refined warm moxibustion pure moxa stick.
2, operation steps
(1) Acupoint prescription Tianshu (double), Qihai, Guangyuan.
(2) Acupoint positioning The patient is placed in a supine position with the abdomen exposed. The Tianshu point is taken in the middle of the abdomen, 2 inches from the umbilicus; the Qihai point is taken in the lower abdomen, on the front median line, 1.5 inches below the umbilicus; the Guangyuan point is taken in the lower abdomen, on the front median line, 3 inches below the umbilicus.
(3) Medicine cake preparation Each cake contains 2.5 grams of medicine powder, add 3 grams of yellow wine, mix into a thick paste, and press with a medicine cake mold into a size of 2.3 cm in diameter and 0.5 cm in thickness.
(4) Moxa cone Use Huatuo brand refined moxibustion pure moxa stick of reliable quality, cut into 1.5 cm long moxa segments to ensure that its size and density reach the same specifications.
(5) Moxibustion method The patient is in a supine position, put the moxa cake on the point to be moxibustion, ignite the upper part of the moxa section and then place the moxa cake on the moxibustion, each time each point is moxibustion 2 strong, each strong about 15 minutes, feel hot when the appropriate movement of the moxa cake.
3.Course of treatment
12 times for a course of treatment, 3 days rest between courses, a total of 6 courses of treatment.
4.Key technical aspects
(1) Patients must be screened in strict accordance with the diagnostic criteria, inclusion criteria and exclusion criteria;
(2) The treatment process should be standardized according to the criteria to prevent the occurrence of adverse events;
(3) The composition of the drug must be made into a uniform specification, be beaten into 100-mesh fine powder, and stored in a sealed container. The drug cake should be made in strict accordance with the formula, and the size of the cake, the drug content and the specification of the moxa segment must be uniform.
5.Caution
During the treatment process, strict operation specification, the medicine cake should be placed smoothly and moved with caution, while paying attention to the patient’s acceptance of warm stimulation, such as burning, should be handled in time to prevent the occurrence of burns.
6.Possible accidents and treatment plan
Burns: remove the moxa cone, apply purple salve, and apply burn ointment.
V. Adverse reactions
Improper operation of moxibustion treatment is prone to burn events, in order to prevent such events, this study, while unifying training to standardize the operation of medical practitioners, made cardboard with holes of the same size as the medicinal cake in the middle, when applying moxibustion, the medicinal cake is placed in the holes and then moxibustion is applied, which can prevent the occurrence of burns.
