Indications for surgical treatment of micro papillary carcinoma of the thyroid

  Surgery is currently recommended for patients with micro papillary thyroid cancer who meet one of the following high-risk factors (indications for surgical treatment of micro papillary thyroid cancer): ① history of radiation exposure to the neck during adolescence or childhood; ② family history of thyroid cancer; ③ established or highly suspected cervical lymph node metastasis or even distant metastasis; ④ extra-glandular invasion (e.g. invasion of the laryngeal nerve, trachea, esophagus, etc.); ⑤ High-risk pathological subtypes (hypercellular subtype, columnar cell subtype, diffuse sclerotic subtype, solid/islet subtype, eosinophilic subtype); ⑥ Positive BRAF gene mutation in the puncture specimen; ⑦ Progressive enlargement of the cancer within a short period of time (more than 3 mm within 6 months).  Relative indications include: ① cancer lesion diameter of 6mm or more; ② multifocal cancer, especially bilateral cancer; ③ patients with high psychological burden and request for surgery; ④ TSH level is persistently higher than normal.
