When it comes to the treatment of clubfoot, it is impossible not to mention the Ponseti method (Ponseti). The Ponseti Method was invented more than 50 years ago by Dr. Ponseti, a pediatric orthopedic surgeon at IOWA University Children’s Hospital in the United States. With the scarring from some of the release surgeries performed by Dr. Ponseti’s team in the 1940s, Dr. Ponseti concluded that surgical correction of clubfoot was the wrong choice. Later, Dr. Pansetti discovered through histological studies of the fetal clubfoot that collagen, a special substance contained in the nascent ligament, is malleable, and this, combined with a thorough understanding of the anatomy of the normal child, adult, and fetal foot, and the interdependence of the joints in the foot. The idea of gradually restoring the deformity to normal by stretching the ligaments that caused the foot deformity, rather than performing surgery, was born, and the correct alignment of the misaligned bones was further improved by light and precise techniques to shape and fix the corrected deformity in plaster. Through the cases they treated, they concluded that it is not difficult to treat clubfoot. After a series of manipulation + cast orthosis, an average of 4-6 casts are applied and usually changed once every 5-7 days. After the high arched foot, forefoot pronation and pronation, and heel pronation are corrected, most of the affected feet require a small minimally invasive procedure – percutaneous Achilles tendon severance – to correct the horseshoe deformity, medically called plantarflexion deformity, also known as dorsiflexion difficulties. A three-week cast allowed the severed Achilles tendon to heal sufficiently in its orthopedic position within the cast. At this point, all deformities are corrected, and when the cast is removed at three weeks the next 4-5 years are spent in braces to maintain orthopedic success. This is also the most challenging step for the parents. Compared to other methods of treating clubfoot, the Pansetti method can overwhelmingly correct all deformities within a month or so to two months, and greatly reduces the scope of surgery, pain and cost of treatment, so it is efficient, economical, easy to master and promote. It has been accepted as the preferred treatment for clubfoot in most countries and regions in the world. From its official introduction to China in 2005 to its official official cooperation and promotion by the Chinese Orthopedic Surgeons Association (COSA) and Pansetti International Association (PIA) in 2015, the Pansetti method has been promoted to a great extent in recent years. Our doctors have the honor to be the first official recommendation, and after the standardized training of Pansetti International Clubfoot Treatment Center, they gradually become the treatment unit chosen by more parents of children with clubfoot.
