Congenital clubfoot is a birth defect disorder that is a common and frequent occurrence. It manifests as a horseshoe deformity of the foot, tension in the Achilles tendon, a small heel, inversion of the ankle joint, and pronation of the forefoot. Delayed diagnosis and treatment can cause serious disability disease. I. Early diagnosis Clinically, there are often doctors in women’s hospitals who find that the baby’s feet are inverted and stiff in the womb when they do 3D ultrasound for the baby; and several times of rechecks show the same changes; and they come to children’s hospitals to find orthopedic specialists to consult with them. So, if you want to talk about early diagnosis, you can actually diagnose it in utero; you just can’t be sure. The family will be very nervous and ask the doctor if they want the baby or not? What will be the future treatment? Will there be any disability? We usually advise them that if other complicated deformities are excluded, if it is just congenital clubfoot, it can be diagnosed at an early stage and treated conservatively, and the result is good; surgery is seldom needed at present. Second, how to be regarded as early treatment? If we can link up with the maternity hospital and confirm the diagnosis after delivery, massage according to Ponsetti’s technique is of course the best choice. However, this is generally not the case. In our clinic, we often see many patients who are already one month old, or even two or three months old, before the initial diagnosis; this delays the optimal time for treatment. Considering the national situation, we suggest that the patient’s family should come to the hospital within a week of the baby’s birth. In this way, we can fight for the patients to the golden time of treatment. Third, early treatment methods One week after delivery treatment, usually the mother postpartum has not recovered can not be accompanied, if you have to be hospitalized to do manipulation massage, playing plaster, the energy of the patient and the family and economic conditions are a test. I now introduce a simple and easy to use, inexpensive, mother and child are not separated from the treatment method. When the child visits the clinic, I will explain in detail the operating points of the manipulative massage and teach the parents how to do the massage themselves. Thanks to the popularity of smart phones, parents are allowed to take photos or videos of the techniques, making it easier for them to learn at home. We will review the treatment in about 10 days to assess the effectiveness of the treatment and to correct the parents’ wrong techniques. Around the third week postpartum, a corrective brace is worn after the manipulation. The brace will be worn in between manipulation and during sleep to consolidate the effect of the treatment. The brace is usually reviewed every three or four weeks. Evaluate the efficacy of the treatment and modify the size, length and angle of correction of the brace. How effective is the treatment? Most of the children return to their normal state after at least two months, or three or four months at the latest. I asked the parents, “Did you work hard? The parents said that it was not hard and their babies did not suffer. I told them that in the past, 80% of the patients with delayed diagnosis and treatment required surgery, but with our current treatment, 90% of them do not need surgery. Individuals in need of surgical treatment, mainly parents with low literacy, either do not grasp the essentials of manipulative massage, or take it upon themselves not to wear orthopedic braces; or negligent management of patients. Fifth, rehabilitation is a long-term process in the baby’s feet back to normal, we have to insist on doing manual massage and wear corrective braces. Generally after the age of one year will walk, conventional conservative treatment can come to an end. In the future, you can wear orthopedic shoes.
