The benefits of fuming steam for chronic prostatitis

The incidence of chronic prostatitis is a common adult male urological disease, according to clinical statistics, the incidence of young men is about 5% ~ 8%, more common in urology and male outpatient clinics. Because of the complex etiology of the disease, the course of the disease is prolonged, seriously affecting the physical and mental health and quality of life of patients, despite the many treatment methods, but the clinical efficacy is not good, there is no satisfactory special treatment methods, bringing confusion to both doctors and patients. The Department introduced NH-QL type Chinese medicine steam therapy instrument produced by Wuxi Huaheng Group Company since 2002, and used Chinese medicine fumigation steam therapy to treat 54 cases of chronic prostatitis patients, and achieved satisfactory results, which are reported below.
  1.Clinical data
This group of 98 patients with chronic prostatitis were outpatients and inpatients of our hospital. The age of the patients ranged from 20 to 55 years old, with an average of 35 years old. All cases were randomly divided into 50 cases of Chinese medicine vapor therapy treatment group (treatment group) and 48 cases of oral prostate Shule treatment group (control group), and there was no significant difference between the two groups in terms of age, etiology, disease duration, and symptoms.
The inclusion criteria of the cases: meet the diagnostic criteria of chronic prostatitis set by the National Institutes of Health (NIH), i.e., symptom score ≥ 4 in NIH chronic prostatitis symptom scale or WBC ≥ 10/HP in EPS, duration of disease ≤ 2 years, age 20~55 years.
Exclusion criteria: combination of other acute diseases, urethral strictures, prostate tumors, serious diseases of cardiovascular, cerebral, hepatic, renal and hematopoietic systems, psychiatric disorders, chronic diarrhea, etc.
  2.Treatment method
In the treatment group, the prescription of Dioscorea soup was adopted, and the medicine was heated in the vaporizer to make the vapor mist of Chinese medicine inside the vapor therapy chamber, and the temperature reached between 38~45℃, and the patients entered the vapor therapy chamber for treatment once a day for 2 weeks. In the control group, the patients were treated with 10g of oral prostate therapy 3 times a day for 2 weeks.
The vapor therapy protocol was prescribed with: Dioscorea Z 10g, Curcuma longa 10g, Cuscuta sinensis 10g, Acorus calamus 10g, Phellodendron sinensis 10g, Stachybotrysum 30g, Boswellia serrata 10g, Myrrh 10g, Peach kernel 10g, Qomai 10g, Tai Wu Yao 10g.
Observation items and methods:To observe the changes of chronic prostatitis symptom index (CPSI) and EPS-WBC examination before and after treatment in both groups.
  3. Efficacy criteria and results
Cured: the symptom score decreased by more than 90% compared with that before treatment, EPS-WBC <10/HP; Effective: the symptom score decreased by 50%~89% compared with that before treatment, EPS-WBC <15/HP or EPS-WBC decreased by 50%~89% compared with that before treatment; Effective: the symptom score decreased by 25%~49% compared with that before treatment, EPS-WBC decreased by 25%~49% compared with that before treatment; Ineffective: the symptom score decreased by 25%~49% compared with that before treatment, EPS-WBC decreased by 25%~49% compared with that before treatment. 25% ~49%; ineffective: symptom level score decreased by 25% or less than before treatment, EPS-WBC decreased by less than 25% than before treatment. Total effective rate = cure rate + significant rate.
Results: In comparison of the efficacy of the two groups, there were 50 cases in the treatment group, 11 cases were clinically cured, 20 cases were apparently effective, 12 cases were effective, 8 cases were invalid, the total effective rate was 60.2%, and the total effective rate was 84%; in the treatment group, there were 48 cases, 5 cases were clinically cured, 12 cases were apparently effective, 10 cases were effective, 21 cases were invalid, the total effective rate was 35.42%, and the total effective rate was 56.23%.
  4.Discussion
Prostatitis is a common urological disease in adult men. In accordance with the new NIH classification criteria, chronic prostatitis is divided into three categories: chronic bacterial, chronic non-bacterial and asymptomatic prostatitis. The most common type of prostatitis is the chronic prostatitis, which belongs to the category of jing (jing), lao (lao) and baixian (baixian) in Chinese medicine. What does it mean? The main cause of the disease is external poisoning, which is the cause of the disease. The main cause of the disease is the external poisonous and hot evil that does not go away, or sexual impurity, damp heat in the seminal chamber, confusing the essence, the essence from its place; the phase fire is strong, because the desire is not fulfilled or to hold back the essence, the kidney fire is depressed and does not disperse, the essence out of place into white turbidity; excessive room to exhaust the essence, the seminal chamber is empty, the damp heat to take advantage of the opportunity to attack the seminal chamber, the essence is forced, can not be hidden. The pathogenesis of the disease is the original deficiency and the symptoms are real. The original deficiency is the weakness of the kidney qi, and the standard reality is that dampness and heat are contained in the seminal chamber; or the spleen is deficient, the transport and transformation is not healthy, dampness and turbidity are generated internally, and the heat turns into heat after a long period of time, which injects into the seminal chamber, resulting in the defeat of seminal injection or overflow, and the development of seminal turbidity; or dampness and heat blockage, the qi is not smooth, and if it does not pass, it is painful, so you can see the urethra, perineum, pubic bone, pelvic floor, lumbosacral area and other parts of the pain and discomfort; dampness is a yin evil, sticky, easy to stay and hurt Yang, and block the qi over time, or cause The most important thing is that it is not a problem.
In contrast, chronic prostatitis (including chronic bacterial prostatitis, chronic non-bacterial prostatitis, prostate pain) has many treatment methods, including microwave, Chinese medicine umbilical therapy, radiofrequency, injection, Chinese medicine sitz bath, etc.; in the treatment of chronic non-bacterial prostatitis, some scholars have reported that treatment with the method of relieving mental tension without any other treatment can achieve satisfactory results; comprehensive psychological and behavioral interventions can also achieve good results.
One of the reasons why chronic prostatitis is so difficult to treat is the plasma-prostate barrier, which reduces the concentration of antibiotics penetrating into the prostate ducts, alveoli and prostate fluid. The traditional Chinese medicine has some advantages in the treatment of chronic prostatitis, but the oral Chinese medicine treatment alone has its limitations.
In addition, the traditional fumigation method of traditional Chinese medicine has certain efficacy in the treatment of chronic prostatitis, but because of the local sitz bath fumigation and simple equipment, the heating temperature of the drug is limited, the active ingredient of the drug has limited volatility, the skin absorption is poor, the temperature of the liquid is unstable, and the action time is short.
The Chinese medicine vapor therapy adopted in our department adds a new high-tech means to the percutaneous drug delivery method of Chinese medicine, which is a new type of thing developed on the basis of traditional Chinese medicine “fumigation therapy”. The temperature of the treatment chamber can be adjusted and maintained, so that different individuals can adopt the most suitable temperature and the therapeutic effect is constant throughout the treatment.
In the formula, Dioscorea Z clears heat and dampness, divides and clears turbidity; Semen Cuscutae tonifies rather than diuresis, benefits the kidney and consolidates essence; Calamus enters the spermatic orifice, opens the ligaments and drains turbidity; Tai Wu medicine benefits kidney qi and helps qi transformation; Huang Bai and Qu Mai clear heat and dampness and lower turbidity; Curcuma longa, Fructus sanguinis, Boswellia serrata, Myrrh and Peach kernel invigorate blood stasis, disperse nodes and open ligaments. The combination of all these drugs can help to clear the turbidity, clear heat and dampness, and activate blood circulation to remove blood stasis, which has achieved good clinical effects.
On the one hand, the drugs are absorbed through the skin after atomization, and the therapeutic drugs can be selected according to the theories of Chinese medicine, which maintains the characteristics of Chinese medicine’s diagnosis and treatment; on the other hand, through the heating effect of the instrument, the temperature of the Chinese medicine vapor can be adjusted and maintained, which maintains the warming effect of Chinese medicine’s external treatment. This treatment method not only conforms to the theory of internal and external treatment of Chinese medicine and the principle of diagnosis and treatment, but also conforms to the principle of modern medicine.
