Vertebral artery type cervical spondylosis is one of the common types of cervical spondylosis, which mostly occurs in middle-aged and elderly people, with people aged 40 to 60 years old being the most common. Modern medicine believes that this disease is a syndrome caused by degenerative changes of the cervical spine, such as osteophytes, hyperplasia of the hook vertebral joint, displacement of small joints and spasm of the neck muscles, which stimulate the extracranial segment of the vertebral artery, resulting in spasm, narrowing or folding changes of the vertebral artery, thus causing insufficient blood supply to the vertebrobasilar artery as the main clinical manifestation. The name of cervical spondylosis does not exist in ancient Chinese medical texts, but is recorded scatteredly in “paralysis”, “spasm” and “vertigo”. In Chinese medicine, this disease is considered to be a deficiency of the original and a symptom of the actual, mostly caused by a combination of deficiency and reality, with the original being a deficiency of the liver and kidney, and the symptom being the attack of wind, cold, dampness and blood stasis, while strain and trauma are also important factors in the development of the disease. The clinical symptoms of this disease are complicated, mostly with vertigo and tinnitus, head and neck pain, plant nerve dysfunction, and in serious cases, sudden collapse and fainting can occur, which is very dangerous to human health. With the popularization of computers, the change of people’s work style and living habits and the acceleration of work pace, this disease is now more frequent and younger [1]. Experts predict that in the next 50 years, this disease will become a modern disease along with the modern society, and will take the place of low back pain, which is mainly caused by physical labor, in the clinical and experimental research of the whole spinal disease and rise to an important clinical position in orthopedics [2]. At present, this disease is mostly used conservative therapy, the author in the clinical use of traditional gua sha with massage techniques for treatment, the efficacy is obvious, try to discuss the following: 1, clinical data A total of 46 patients, 27 male cases, 19 female cases; the minimum age of 38 years old, the maximum age of 66 years; the shortest duration of 5 months, the longest 7 years. All patients in this group had dizziness, headache, neck discomfort, tinnitus, and blurred eyes, including 2 cases of blurred vision, 5 cases of episodic dizziness, nausea and vomiting, 3 cases of insomnia and anxiety, and 1 case of sudden fall with clear consciousness. 42 patients showed different degrees of vertebral redundancy and narrowing of intervertebral foramina on X-ray, and 4 cases of cervical spine CT showed cervical disc prolapse. All patients were excluded from serious cardiopulmonary disease, tuberculosis, tumor and spinal cord type cervical spondylosis. As a result, 25 cases (54%) were cured, 15 cases (33%) were effective, and 6 cases (13%) were effective, with an overall efficiency of 100%. There is no significant difference in the efficacy between pure vertebral artery type cervical spondylosis and mixed type cervical spondylosis (mainly vertebral artery type combined with other types). 4, Discussion 4.1 Scraping first, accurate diagnosis Scraping is a traditional Chinese medical therapy, easy to operate, in this disease its first role is to play the role of auxiliary diagnosis: where the cervical petechiae, especially the petechiae are obvious, most of the pathological nodules and vertebral pathological changes [4], can provide positioning reference basis for the subsequent massage operation. The Gua Sha performance can also reflect the nature and general prognosis of the disease. In general, Gua Sha is bright red and dotted mostly for superficial evidence, short duration of illness, light condition, and good prognosis; Gua Sha is dark red, patchy or stasis block mostly for internal evidence, long duration of illness, heavy condition, and poor prognosis; with treatment, the color of Gua Sha spots change from dark to red, from patches to scattered spots, indicating that the condition is improving [5]. 4.2 Gua sha in place and therapeutic effect revealed Of course, gua sha also plays an important therapeutic role in this disease. Gua sha is a therapy based on meridian theory, and its treatment of disease is based on the principle of dispelling wind and cold, invigorating blood and eliminating blood stasis, and activating meridians. Gua sha can invigorate the yang energy of the meridians, expel the pathogens that block the meridians and stagnate the channels from the body surface, regulate the qi and blood, improve the blood supply to the nerves, improve microcirculation, create a highly congested state in the local tissues, promote the absorption of edema, accelerate the excretion of acidic metabolites, and reduce the stimulation of pain-causing substances on the nerve roots [6]. Another study confirmed that the physical stimulation of gua sha reduces the excitability of nerve endings and decreases the signaling to the center, so that the local tissue response to pain gradually diminishes [7]; gua sha can cause local stasis of blood, leading to self hemolysis and the formation of a new stimulant that can strengthen local metabolism and has an anti-inflammatory effect [8]. In the author’s clinical practice, there are often patients who complain of a great reduction in symptoms before the end of a single scraping treatment. 4.3 Tui Na assistance, deep penetration lesions As mentioned earlier, Gua Sha has a further clear diagnosis and plays a practical therapeutic role in this disease, and can also save the operation of conventional relaxation techniques for the subsequent Tui Na treatment. However, the lesions are deep and variable in location, so gua sha is always a large area operation, and it is difficult to penetrate deeply enough to reach the lesions. The precise and accurate massage technique can achieve the effect of “deep penetration”, so as to promote the circulation of qi and blood, dredge the meridians and tendons, and open the joints. The rotational techniques can also help to comb and loosen the deep tissues such as joint capsule, intervertebral ligament and synovial membrane [9]. In addition, it has been found that the head and neck manipulation can relieve the tension of sympathetic nerves around the vertebral artery, eliminate the spasm of the vertebral artery and restore the normal blood supply of the vertebrobasilar artery by the action of nerve reflexes, so as to achieve the purpose of treatment[10] . 4.4 Health education and consolidation of the therapeutic effect “the upper work treats the untreated disease”, we should diligently do health education and explain that patients should pay attention to the regulation of care in life and work in general to consolidate the therapeutic effect and prevent recurrence: we should avoid prolonged low head posture; we should correct some bad habits in life, such as half lying in bed watching TV; choose suitable pillows and The patient should be taught to practice cervical spine health care exercises and asked to adhere to the exercise that is beneficial to the neck such as badminton and swimming.
