Abstract: A 32-year-old female patient attended a health checkup and all tests were normal, but a gynecologic ultrasound examination suggested a slightly hyperechoic uterine cavity of about 0.5×0.2×0.3 cm.
Basic information】Female, 32 years old
Disease Type】Endometrial polyp
Hospital】Hunan Provincial Maternal and Child Health Hospital
Date of consultation】May 2022
Treatment plan】Surgical treatment (hysteroscopic endometrial polyp electrosurgery, intrauterine placement of the Mannedol ring) + medication (estradiol valerate tablets)
Treatment period】Day surgery for 1 day, review after 1 month
Results] Endometrial polyps were removed and the patient recovered well after surgery.
I. Initial consultation
At the first visit, the patient was in good spirits and had a steady gait. She reported that she had regular menstruation for 5-6 days/28 days and no dysmenorrhea. Last month, she went to an outside hospital for physical examination and found that all examinations were normal, but gynecological ultrasound examination suggested that the intrauterine cavity was slightly hyperechoic about 0.5×0.2×0.3 cm, so she visited the hospital for further examination and treatment. After communication with the patient, we improved the relevant examinations: the endometrium was about 0.83 cm thick, and most of the echogenicity in the upper middle segment was slightly high, with localized masses and uneven distribution; CDFI: no significant blood flow signal was seen in the uterine cavity. The preliminary diagnosis was endometrial polyp, and the patient had symptoms of increased menstrual flow.
II. Treatment history
After the patient was admitted to the hospital, routine preoperative examinations such as routine blood, liver and kidney functions, routine leukocyte examination and electrocardiogram were performed, and the results were found to be normal. After signing the consent form, the patient was sent to the operating room for preoperative disinfection and other preparations. After general anesthesia, the hysteroscope was inserted into the vagina and uterus. The cervical canal was seen to be regular in shape, and a small amount of tissue was scraped out. Postoperatively, oral estradiol valerate tablets were prescribed to prevent uterine adhesions.
III. Treatment results
The patient regained consciousness within 10 minutes after surgery and was in a good mental state, with no nausea, dizziness or other uncomfortable symptoms upon inquiry. The patient was asked to walk on the floor with the support of her family. She talked with the patient clearly and logically, suggesting that the operation was successful and there were no other uncomfortable symptoms.
IV. Precautions
I am sincerely happy for the patient because of the good recovery and no obvious abnormality in pathological examination after the surgical treatment.
1, daily rest should be paid attention to ensure sufficient sleep, avoid staying up late, overworked.
2, strengthen nutrition, choose foods rich in vitamins and proteins, such as fresh vegetables and fruits, eggs, milk, etc., avoid spicy and stimulating diet during the recovery period.
3, after the operation should pay attention to keep the vulva clean, diligent replacement of intimate clothing, 1 month prohibited sex and tub bath, in addition, after the IUD may appear the symptoms of drip bleeding, but generally after six months can be self-relieved;.
4. Pay attention to your own changes after the operation, and seek medical attention promptly if symptoms such as abdominal pain and fever appear.
V. Personal insight
The patient was found early and the polyps were small because of her strong health awareness and timely medical review after the abnormalities were found in the physical examination. I am glad that she cooperated actively with the doctor after consultation. The patient’s pathological examination results did not show any malignant lesions and the outcome was good. We would like to urge all women to seek medical examination if they have abnormal menstrual flow, abnormal vaginal bleeding, or if they only find abnormalities in physical examination, as early diagnosis and early treatment can effectively improve the prognosis and promote the health of the body.
