What are the main roles of cleft lip nasal molds

Many times after cleft lip surgery, doctors will recommend that the child’s family wear a nasal mold for the child. What does it do? How long should it be worn? The following is an introduction to each of them. A cleft lip nasal mold is a medical silicone product made according to the normal shape of the nasal vestibule, which can be worn after cleft lip nasal deformity surgery to induce the normal development of the external shape of the nasal vestibule. The role of cleft lip nasal mold is mainly as follows: (1) During the revision of cleft lip nasal deformity, extensive separation will be done between the nasal cartilage and the skin, the purpose of the separation is to allow the misplaced tissue to reattach in its normal position. During the surgery we will fix sutures at some important loci, and the application of cleft lip nasal mold can stabilize the effect of reattachment after correction.        (2) The lining of the cleft lip nasal mold allows the elimination of the widely separated cavity between the nasal cartilage and skin in order to avoid excessive hypertrophy of the nasal vault in the future. (3) The nasal cartilage morphology of cleft lip nasal deformity is not arched as in normal children, but mostly flattened. And in the early stage of the child, the nasal cartilage is plastic. Taking advantage of this physiological feature, the application of cleft lip nasal molds can turn the original flat nasal cartilage into an arch shape with normal curvature. This is particularly important because the curvature of the nasal cartilage has a significant impact on the shape of the nose and it is very difficult to change the curvature of the nasal cartilage later on by surgery. The longer the nasal mold is used, of course, the better, but it still depends on the care of the parents and the cooperation of the child. The average wearing time is 6 to 12 months. Generally speaking, children will start to move their nasal molds around 9 months to 1 year old and refuse to wear them. However, most of the children can wear it until they are about 1 year old, and during the follow-up, we found that the nostrils change the most around 1 year old, and if they don’t continue to wear it, the nostrils on the affected side will start to collapse and the deformity will get worse. Therefore, it is very important to keep wearing it.
