A patient with persistent hiccups with episodes of body tightness was recently encountered in the outpatient clinic. Several visits to gastroenterology and respiratory medicine were made, and diseases related to digestion and respiration were ruled out. Many symptomatic medications were taken without significant improvement, and a neurological consultation was recommended. After ruling out epilepsy and other seizure-related neurological disorders, the patient’s thyroid function was examined and hyperthyroidism was found. After anti-thyroid treatment was given, the patient’s erratic symptoms and body tightness improved significantly. The pathophysiological mechanism of hyperthyroidism causing erratic reflux is not fully understood. Most researchers believe that abnormal thyroid function can lead to abnormal metabolism of the dopamine system in the basal ganglia of the brain. In addition, hyperthyroidism is an autoimmune disease, and the immune mechanism leading to brain damage cannot be excluded. In conclusion, it is advisable to have thyroid function tests for patients with intractable erratic symptoms.
