Clinical manifestations
The basic lesion first appears as a round or oval, pale red or yellowish red scaly spot about finger cap or slightly larger somewhere on the trunk and extremities, called the primary spot or mother spot, which is easily ignored by patients. One to two weeks after the appearance of the mother spot, that is, most of the erythematous spots with the same shape as the mother spot but smaller appear on the trunk and proximal extremities, called daughter spots or secondary spots. The lesions are transverse and elliptical, the long axis is basically the same as the skin line, the center is slightly wrinkled, the boundary is clear, the edge is not neat, slightly jagged, and the surface is accompanied by a small amount of chaff-like fine scales. Most of them are isolated and do not fuse with each other. After the appearance of the daughter spots, the mother spots are darker in color. The color of the rash varies from bright red to brown, brownish yellow or grayish brown. The lesions are usually found on the chest, back, abdomen, proximal extremities, and neck, especially on both sides of the chest, and a few are also seen on the upper part of the femur, but the face and lower legs generally do not occur, and the mucous membranes are occasionally involved. Patients have varying degrees of pruritus, and some patients may initially have generalized symptoms such as peripheral discomfort, headache, sore throat, mild fever, and swelling of the neck or axillary B-nuclei.
The prognosis of this disease is good, generally about 4-6 weeks can naturally subside, the skin back to normal, does not leave any trace; there are also delayed 2-3 months, or even longer time to heal. After healing, there is usually no recurrence.
Differential diagnosis]
1.Floral pityriasis: Chinese medicine called purple vitiligo, sweat spot. Mostly in the chest and back, the side of the neck, scapula, etc.. The lesions are yellow bean to bean size patches, slightly shiny, first light red or red purple, will be healed gray-white patches, the surface has fine scales. The fungal microscopy is positive.
2. Tinea corporis: equivalent to ringworm. The damage is generally small in number, although circular in shape, with clear borders and a tendency for the center to heal itself, with a flaky surface and surrounded by papules and small blisters. Fungal microscopy is positive.
3, psoriasis: equivalent to white H, people often called psoriasis. The lesions are erythematous, with thick silvery-white scales on the maculopapular rash, with film phenomenon after scratching and punctate bleeding, with a long disease course and easy recurrence.
4, syphilis: caused by syphilis spirochete, the second stage of syphilis rash first in the chest, and then gradually to the waist and abdomen, the flexors of the limbs, face and neck, and finally the hands, the early rash is about 0.5cm diameter round or oval light red spots, each independent, not fusion, but also into a part of the appearance of papules, scaly papules and pustules, etc.. The palmoplantar can be seen as a flaky rash, the mucous membrane can appear mucous membrane spots, vulva and anus can occur flat warts, hair can be worm-like loss, superficial B nucleus enlargement. It can last for 1~2 months without treatment, and will rapidly subside after anti-syphilis treatment.
Treatment
A, Chinese medicine diagnosis and treatment
1, wind-heat skin type: the onset is rapid, the lesions are round or oval light red red patches, the center has fine wrinkles, the surface of a small amount of furfuraceous scales quiet.
