It is generally accepted that body mass index, BMI, can represent the degree of obesity of a person, BMI=weight/height2 (Kg/m2), BMI25~30 is overweight, BMI≥30 is obese, BMI≥40 is severe obesity. Obesity is a recognized factor related to surgical risk and postoperative risk, and patients with severe obesity put forward super high demands on surgery, anesthesia and nursing care. Recently, our hospital successfully completed a weight reduction surgery for an extremely obese patient with a BMI of 62.5 Kg/m2, which is probably the highest BMI in Shanghai at present. Patient Xiao Ding (pseudonym), female, 29 years old, was admitted to the hospital because of “weight gain for 24 years, menstrual disorder for 6 years, amenorrhea for 5 years”, the patient started to gain weight gradually after she was 5 years old, weighing 65kg at the age of 12, reaching 85kg at the age of 19, and gaining weight significantly after pregnancy, increasing about 9kg per year, 159kg at the age of 27. At the age of 27, she was 159kg, tried to lose weight, lost 35kg, rebounded after 2 months, shortened menstrual cycle to 15 days since the age of 23, stopped menstruation 5 years ago, extremely poor sleep quality, at the time of admission, the patient was 4000px in height, 160kg in weight, 4000px in waist circumference, BMI 62.5Kg/m2, extremely severe obesity. It is noteworthy that the patient was treated with a ventilator for lung infection and respiratory failure 1 year ago. The patient currently has severe sleep apnea, elevated blood glucose, menstrual disorders, which seriously affects her quality of life, and after multidisciplinary discussion, it was unanimously decided that she should be operated on for weight loss. With the full cooperation of the anesthesiology department, the operating room and the gastroenterology endoscopy room, the operation went exceptionally well and lasted 1 hour and 30 minutes. Xiao Ding’s stomach was laparoscopically cut into a tube (about 150 ml capacity), while the excess stomach was removed from the abdominal cavity. He started to drink water 1 day after the operation, ate fluids for 3 days, and was discharged from the hospital 1 week later. With the improvement of living standard, more and more “super fat” people emerge in China, for which medical treatment is almost ineffective, and obesity brings a lot of inconvenience to life and work, and even fatal. In this case, the patient was extremely obese, with a BMI of 62.5 Kg/m2, which is three times that of an ordinary person, and her life was already in danger.
