The characteristics and advantages of TCM treatment are dialectical treatment, the combination of dialectical evidence and disease, that is, different treatment of the same disease and different treatment of the same disease. It is appropriate to balance the yin and yang of human body, and the treatment of water is also appropriate to dredge to guide, and should not be obstructed or blocked, otherwise it will stay stagnant as a problem, which is more harmful than others. Therefore, there are many treatment methods for macular edema, but I think there are mainly the following: 1, strengthen the spleen and water method – the symptoms are blurred vision or visual deformation becomes smaller, macular edema cloudy, the central reflection disappears. It is often accompanied by poor appetite, loose stools, thin or slippery coating and thin pulse. Representative formula: Wu Ling San plus reduction (Atractylodes macrocephala, Poria, Poria, Zelig, Gui Zhi, Rice kernel, Yam, Scutellaria), commonly used in “middle pulp” and “middle ooze” diseases. 2, activating blood circulation and resolving blood stasis combined with the method of water circulation – the symptoms include sudden decrease in visual acuity, more bleeding in the fundus of the eye, cloudy macular fluid, often accompanied by headache and dizziness, or numbness of the limbs, red tongue often with petechiae, and string pulse. Representative formula: Blood Mansion and Blood Stasis Soup plus reduction. (Radix Angelicae Sinensis, Rhizoma Chuanxiong, Radix Shengdi, Radix Achyranthes Bidentatae, Radix Paeoniae Alba, Rhizoma Peach Blossom, Radix Platycodon Grandis, Radix Bupleurum, Radix Platycodon, Atractylodes Macrocephala, Poria, Radix Zedoariae, Panax Ginseng Powder) Commonly used in retinal vein embolism, retinal vasculitis, diabetic retinopathy and other diseases. 3.Liver clearing and diarrhea of fire—Symptoms include blurred vision or deformation of vision, edema and hemorrhagic spots in macula, or accompanied by congestion and edema of optic disc, eye swelling and pain, pain in the mouth, red tongue with yellow fur, and string pulse. Representative formula: Gentian and liver diarrhea soup with addition and subtraction. Commonly used in diseases such as “middle seepage” and optic neuritis. 4.Drain the liver and lower the fire combined with the method of diuresis–See blurred vision, edema and exudation in the macula or edema and congestion in the optic disc, retinal hemorrhage, irritability, bitterness and dryness of the mouth, dark red tongue, thin coating and thin pulse. Representative formula: Dan Gardenia Easy-going San plus or minus. (Chai Hu, Radix Angelicae Sinensis, Radix Paeoniae Alba, Mouton, Poria, Atractylodes Macrocephala, Radix Glycyrrhiza Uralensis, Dampi, Gardenia, Zedoary, Plantago). It is commonly used in “middle seepage”, “middle pulp”, optic neuritis and other diseases. 5, phlegm and stasis with the treatment method – the symptoms are long-lasting, persistent, macular exudate edema is obvious, often accompanied by macular cystic edema, obese, heavy head dizziness, greasy tongue coating, red tongue texture or petechiae, string or slippery pulse. This is a case of “prolonged illness with more stasis”, “persistent illness with more phlegm”, and interconnection of phlegm and stasis. Representative formula: Peach-Hong Siwu Tang combined with Wenzhi Tang plus reduction. (Peach kernel, safflower, angelica, chuanxiong, raw earth, white peony, bamboo and half asia, poria, Chen Pi, Jackfruit shell, raw hawthorn, zhebei mother, party ginseng). It is common in diseases such as retinal vein embolism, age-related macular wet degeneration, diabetic retinopathy, etc. with a long course of disease. In conclusion, macular edema is a common sign of many diseases. It seriously affects the visual function of patients, which should attract our great attention and actively explore its treatment. Mr. Chen Dafu in “The Six Scriptures of Chinese Medicine Ophthalmology” believes that “the macula is yellow in color and resides in the center of the optic garment, when it is the essence of the internal organs.” In his “Internal Eye Tissues and Viscera Meridians and Collaterals,” he pointed out that “the macular area of the retina belongs to the foot Taiyin Spleen meridian, and at the same time, it should take into account the foot Convulsive Yin Liver meridian to which the retina belongs.” Mr. Zhu Danxi in “Danxi Xinfa? Phlegm” states, “Phlegm carries blood stasis and thus becomes an areolar sac. Professor Wei Qi Ping of Beijing University of Traditional Chinese Medicine believes, “Whatever the cause of macular edema, eliminating stasis, resolving phlegm and reducing swelling is its basic treatment.” All the above sages’ discourses are worthy of our study, discussion and practice.
