Achalasia Definition: Pain on the metatarsal surface of the heel bone caused by multiple chronic disorders. Diagnosis: 40-60 years old, pain on walking. Examination: localized pressure pain (+), which can be caused by the following pathologies: 1. fat pad atrophy: pressure point in: the weight-bearing area of the heel is medial, and sometimes a subcutaneous fatty fibrous mass can be palpated; 2. metatarsal fasciitis: pressure point in: the metatarsal fascia of the greater tuberosity of the heel; 3. heel bursitis: pressure point in: under the medial tuberosity of the heel. X-rays: sometimes bone spurs are visible, but most of them are not significantly correlated with clinical manifestations. Treatment: 1, mostly conservative treatment: (1) reduce weight bearing, such as standing, walking; (2) custom arch insoles; (3) local heat, physical therapy, local closure: local warmth, hot compresses; (4) drug application: white vein ointment (topical), pain and blood capsule application. (2) Surgery: metatarsal fascia stripping, bursal resection, heel bone drilling, neurectomy.
