With the development of society and changes in lifestyle and work style, and the widespread popularity of computers, cervical spondylosis has a rapid increase and a trend of rejuvenation, which seriously affects our quality of life and work efficiency.
(A) What are the common manifestations of cervical spondylosis?
Due to the complexity of the structure and function of the cervical spine and surrounding tissues, the manifestation of cervical spondylosis is also very complicated.
You can feel yourself, and if you have the following symptoms, then you may have cervical spondylosis
(1) Pain or soreness at the back of the neck, accompanied by unfavorable neck movement, recurring episodes, which can be alleviated by tugging the head and neck upward by hand, but aggravated by putting pressure downward.
(2) Pain in the neck is accompanied by radiating pain or numbness in the arms (including the hands), and the symptoms are aggravated when the head and neck are stretched back or moved to the opposite side of the affected side repeatedly.
(3) When the head and neck are rotated from side to side, migraine or vertigo can be triggered or aggravated. There have been cases of fainting due to rotation of the head and neck.
(4) Pain in the neck is accompanied by weakening of muscle strength in the upper or (with) lower limbs, atrophy of limb muscles, and pain in the limbs.
(5) When the head is lowered, the whole body suddenly becomes numb or has an “electric shock”-like sensation.
(2) How many types of cervical spondylosis are there?
According to different pathological changes, cervical spondylosis can be generally divided into several types, such as cervical cervical spondylosis, neurogenic cervical spondylosis, spinal cord cervical spondylosis, vertebral artery cervical spondylosis, sympathetic cervical spondylosis and mixed cervical spondylosis.
1. Cervical cervical spondylosis.
The main manifestations are pain and discomfort in the head, neck, shoulder and back, restricted activities, often feeling that the neck is easily fatigued and cannot read books or watch TV for a long time; sometimes headache, pain in the back of the occiput, or “neck tightness” and “stiffness” in the morning after waking up, and inability to move or when moving There is a ringing sound in the neck.
2.Nerve root type cervical spondylosis.
The main manifestation is neck and shoulder pain, accompanied by radioactive pain and numbness in one or both upper limbs, and the pain can be triggered or aggravated when coughing, sneezing, stretching the upper limbs and overflexion or overextension of the neck, and the pain can affect sleep in severe cases. In severe cases, muscle atrophy and weakness can occur.
3.Spinal cord type cervical spondylosis.
Spinal cord type cervical spondylosis is a heavier type, and the symptoms are initially numbness and weakness of the lower limbs, difficulty in lifting the legs, unstable gait, weak legs, easy to fall, and a feeling of stepping on cotton on the soles of the feet when walking. Later, numbness, swelling, burning or coldness, pain or weakness in the upper extremities, holding objects easily, shaking, and clumsy hands when performing fine activities (e.g., writing, fastening buttons, etc.). In severe cases, urination and defecation are impaired, and perianal or perineal sensation is abnormal.
4.Vertebral artery type cervical spondylosis.
The main manifestations are pain in the head and occipital area, dizziness caused by large neck rotation and posterior extension activities, nausea, headache and loss of vision, tinnitus, deafness, and even sudden fall due to dizziness, recovery after a few seconds of sudden fall, no impairment of consciousness, and no sequelae. At the same time, there may also be memory loss, forgetfulness, poor sleep and dreaminess, easy to be frightened and other accompanying symptoms.
5.Sympathetic cervical spondylosis.
Sympathetic cervical spondylosis has complex manifestations, including headache or migraine, dull head, dizziness, occipital pain or posterior neck pain, enlarged eye fissures, blurred vision, dilated pupils, swelling and pain in the eye sockets, dry eyes, and gold stars in the eyes, accelerated heartbeat, disturbed heart rhythm, anterior heart pain and increased blood pressure, and excessive sweating.
People should go to the hospital promptly when the above performance occurs.
(C) What are the methods for treating cervical spondylosis?
There are various clinical applications for the treatment of cervical spondylosis, which can be summarized as non-surgical treatment and surgical treatment. Non-surgical treatment mainly includes western medicine, Chinese medicine, cervical braking, traction, manipulation, physical therapy, acupuncture and transcutaneous electrical stimulation, functional exercise, etc. Surgical treatment can be divided into conventional incisional surgery and minimally invasive surgery. The vast majority of patients with cervical spondylosis are cured or relieved by non-surgical treatment, and spinal cervical spondylosis is the main type of surgery.
Traditional Chinese medicine has the advantages of variety, obvious curative effect, easy operation, low price, high safety and integration of prevention, treatment and rehabilitation. Traditional Chinese medicine has six advantages in treating cervical spondylosis: ①Diversity: Chinese medicine, massage, acupuncture, functional exercise and other therapies are diverse and have obvious characteristics; ②Effectiveness: recent studies have shown that Chinese medicine is effective in treating the disease, with an efficiency rate of more than 90%; ③Simplicity: easy to operate, low site and equipment requirements, low price, suitable for promotion; ④Safety: Chinese medicine methods are more effective than chemical drugs, toxic side effects and surgery. ⑤ Complementarity: various TCM methods can be used together, and can also be used as remedial measures before and after surgery, or when treatment fails or is ineffective; ⑥ Sustainability: long-term treatment with TCM in all stages can be prevention and treatment, so that prevention, treatment and rehabilitation are mutually beneficial.
We focus on the patient is most willing to accept, the most widely used in clinical practice, and the most controversial massage therapy. Chinese medicine manipulation has a long history and is widely used clinically, and is a common treatment method in orthopedics and traumatology, and is also one of the main clinical treatment methods for neurogenic cervical spondylosis. However, manipulation is skillful and affects the therapeutic effect due to the lack of standardization, and adverse reactions have been reported; there is a lack of multicenter, randomized controlled clinical studies on the evaluation of the efficacy and safety of manipulation, which is mostly limited to general experience summary at present; basic research on manipulation is lagging behind, and the mechanism of action is mostly speculative. Compared with the relatively standardized acupuncture therapy, which also has Chinese medical characteristics, this has become a key issue affecting the efficacy and safety of manipulation and restricting its application at home and abroad.
We have accumulated many years of clinical experience, and through the national “Tenth Five-Year Plan”, “Eleventh Five-Year Plan”, the National Natural Science Foundation of China project and many other studies, according to the theory of “tendons and bones” in Chinese medicine. Combined with the characteristics of traction and inherited the traditional rotational technique, we established and verified the rotational lifting technique and its operation specification. The technique has been applied in 30 hospitals nationwide, treating more than 50,000 patients, with an efficacy of more than 90% and no adverse reactions.
(D) What should be noted in the prevention and rehabilitation of cervical spondylosis?
(1) Psychological care.
Patiently explain the causes and treatment methods of cervical spondylosis to patients, so that they can put aside their worries and actively cooperate with the care.
(2) Healthy lifestyle guidance.
(1) Pillow and sleeping posture: Pillow should be suitable, use round pillow. The height of the pillow is generally 11-15 cm, and the pillow core is soft and hard to be comfortable. The pillow should be placed at the back of the neck, keeping the head lightly tilted back posture, so that it conforms to the physiological curvature of the cervical spine. Side lying pillow should be the same height as the shoulder, keep the head and neck at the same level; ② work posture: sitting work should try to avoid hunching, head down, not to write on the table, do not overly low when reading, try to keep the book and eyes parallel. Reading, writing, using the computer, driving a car, etc. should not be too long, generally work 45 minutes to do 1 to 2 minutes of head and neck activities or change the posture; ③ daily life and housework: walk with chest up, eyes flat in front, do not lie in bed and read, manual labor, watching TV, do not overly low, the time should not be too long, and often change the posture.
(3) Food therapy.
① kudzu barley five plus porridge.
Ingredients kudzu, barley, japonica rice, 50 grams each, 15 grams of prickly five plus. Method of raw materials washed, chopped kudzu root, stinging five plus first decoction to get juice, and the rest of the ingredients together in a pot, add the right amount of water. Boil on a high flame and simmer on a low flame to form a porridge. Can add rock sugar appropriate amount. Useful for dispelling wind, removing dampness and relieving pain. Indicated for wind-cold-damp paralysis type cervical spondylosis (heavy head, stiff neck, unfavorable movement, cold and wind).
② Papaya and Chen Pi porridge.
Ingredients papaya, peel, loofah, Chuanbei mother 10 grams each, 50 grams of round-grained rice. Method raw materials washed, papaya, peel, loofah first decoction, remove the dregs to extract juice, add Chuanbei mother (chopped), add ice sugar to the appropriate amount. It is suitable for cervical spondylosis (dizziness, dizziness, heavy head like wrapping, numbness of limbs and dullness).
③ Strong bone soup.
Raw materials pig bone (preferably pig tail bone) 200-300 grams, Eucommia, Fructus Lycii 12 grams each, cinnamon meat 15 grams, cow knee 10 grams, Chinese yam 30 grams. Method raw materials washed, chopped pig bones, into a pot, add the right amount of water, boil on a high flame, decoction 40. 60 minutes, add the right amount of peanut oil, salt, onions, ginger and other ingredients, take the soup. Indicated for cervical spondylosis (dizziness and headache, tinnitus and deafness, insomnia and dreaminess, numbness of limbs, muscle atrophy, soreness and weakness of the waist and knees).
④ Ginseng and jujube porridge.
Ingredients: 3 grams of ginseng, 50 grams of round-grained rice, 15 grams of jujube. Method ginseng crushed into fine powder, rice, dates washed into a pot, add water to the appropriate amount, boil, simmer into porridge, then mix in ginseng powder and sugar appropriate amount. It is suitable for cervical spondylosis (dizziness, pallor, palpitation, shortness of breath, numbness of limbs, tiredness and fatigue).
(4) Functional exercise.
Recommended exercises include swimming, kite flying, badminton, Taijiquan, Badaunjin, etc. The following principles should be followed when exercising.
① Implement the principle of “tilting the head and raising the arms, coordinating and balancing” to exercise the posterior extension muscles of the neck and balance the neck stress and stable balance disorder caused by long-term low head position.
② Practice should be done without aggravating the original disease. To carry out local practice, special attention should be paid to not aggravate the local tissue damage, strengthen favorable activities and avoid unfavorable activities. According to the different conditions, choose the appropriate practice movements to stipulate the amplitude and number of times of practice before the neck practice, so as not to aggravate the disease.
③ The amplitude of practice movements should be from small to large, the number of times from small to large, the time from short to long, the strength from weak to strong, and the movements from simple to complicated, so that the pain is not aggravated when practicing, or a slight reaction can be tolerated.
④ Hands-on exercise and active exercise should be the main focus, supplemented by equipment exercise and passive exercise.
The following is another set of recommended cervical rehabilitation exercises.
① Forward flexion and backward extension.
Standing position, neck and shoulders relaxed, cervical spine slowly upward stretch, slowly forward flexion, up to the maximum, static force to maintain 5 seconds, and then return to neutral position; cervical spine slowly backward, up to the maximum, static force to maintain 5 seconds, and then return to neutral position. Repeat this 10 times.
② Rotate the neck and look at the heel.
Standing position, feet apart and shoulder width, hands naturally down, neck and shoulders relaxed, cervical vertebrae slowly upward stretching, head and neck left rotation, eyes to the back of the bottom to look at the heel of the opposite side of the foot, in the maximum force to stretch the neck, hold about 5 seconds, restore the right side to repeat the same action. Repeat this 10 times.
③ back to look at the moon.
Standing position, feet apart and shoulder width, hands naturally down, neck and shoulders relaxed, cervical vertebrae slowly upward stretch, head and neck left rotation, eyes to the left side after 45 degrees look out, in the maximum force to stretch the neck, hold about 5 seconds, restore the right side to repeat the same action. Repeat this 10 times.
④ Chicks taking off.
Standing position, feet apart and shoulder width, hands held behind the back to stretch backward, shoulders up, while the head and neck slowly upward stretch, try to lean back, neck and shoulder back muscles contracted hard to maintain 5 seconds, neck and shoulder muscles relaxed to restore the neutral position. Repeat this 10 times.
⑤ Shake and turn your shoulders.
Stand and sit with your hands hanging down naturally, while shaking both shoulders slowly 10 times from neutral position to backward, backward, forward and forward to neutral position, and then shake slowly 10 times from forward to backward in the opposite direction.
Each exercise lasts 5 minutes, adhere to 2 times a day.
(5) their own massage.
① Select points.
Fengchi point, in the recess below the occipital bone; complete bone point, touch the back of the earlobe, there is a convex bone called “mastoid”, from below the bone along the back edge, there is a shallow concave is; Tianzhu point, in the lower concavity of the back of the skull, that is, a protruding muscle at the neck (oblique muscle), the outer concavity of this muscle, after the hairline is open about 2 This is the point.
Shoulder well point, in the center of the shoulder peak and the first thoracic vertebrae link;? The wrist bone point is located between the fifth metacarpophalangeal joint and the wrist joint on the outside of the palm, about 1/3 of the way to the wrist, and the above points produce a soreness and swelling when pressed.
② Pointing. Use the tip of the thumb or index finger as a technique, starting with the wrist bone, and then pointing the shoulder well, Fengchi, Wanbone, and Tianzhu points in turn. First, press lightly and gradually for 3 to 5 minutes, and then knead the points clockwise for 3 to 5 minutes, once a day in the morning and once in the evening, for 10 minutes each time, and persist for a long time.
In addition, arm numbness, can be rubbed along the upper arm, forearm, and point press Quchi, Hegu and other points; dizziness, headache, can be used to gently tap the five finger tips of both hands on the head. Get up once a day in the morning for 15 minutes each time.
