The shoulder pain that is characteristic of patients with hemiplegia is called hemiplegic shoulder pain. The degree of pain varies widely. Some patients experience pain when the shoulder joint moves beyond a certain range, others experience pain when they move, and still others experience severe pain when the shoulder joint is touched. There are several common causes of hemiplegic shoulder pain: during the acute stage of stroke, the paralyzed upper limb is very flaccid and the joint ligaments of the shoulder cannot bear the weight of the upper limb, resulting in shoulder joint subluxation and pain; the paralyzed arm cannot move, resulting in poor blood circulation and joint adhesions at the shoulder joint, resulting in pain when the arm is moved again; when moving the patient or performing rehabilitation exercises, the range of activity is too large, causing joint and muscle damage, resulting in shoulder pain, etc. When moving the patient or carrying out rehabilitation exercises, excessive movement can cause damage to the joint and muscles, resulting in shoulder pain, etc. The prevention of hemiplegic shoulder pain has a great impact on the recovery of the patient’s entire upper limb. First of all, the correct position should be maintained when lying down, especially when lying on the side to prevent pressure on the paralyzed limb. However, when helping the patient to move the shoulder, use one hand to protect the shoulder and the other hand to move the shoulder in a small to large range to prevent the shoulder joint from dislocating due to excessive amplitude. In addition, acupuncture, rehabilitation passive exercise, massage and physical therapy can also be performed. A lot of clinical practice has proved that the earlier the cerebrovascular disease attack, the sooner the acupuncture and rehabilitation measures are carried out, the more likely the hemiplegic shoulder pain attack will be reduced or the attack will be less severe. Therefore, as long as the condition allows, the sooner the rehabilitation measures for cerebrovascular disease are carried out, the better.
