Treatment of neurodermatitis

  Neurodermatitis is one of the more common pruritic diseases of the skin and the standard name of the disease is: chronic simple moss. It is associated with many internal and external factors such as neuropsychiatric factors, alcohol consumption, and local irritation. The vicious cycle of itch-scratch-exacerbation of itch formed during the course of the disease is an important cause of the development of the disease and leads to localized mossy skin lesions.  The common clinical manifestations of neurodermatitis are: thickening of the skin striae in the neck, upper lid, elbow, ankle, lumbosacral, vulva, etc., and the appearance of polygonal flat papules of varying sizes of skin color or light brown, light red or uneven pigmentation, with conz-like scales on the surface, with obvious itching, sometimes persistent itching. The itching is significantly increased by various stimuli and irritation, and the rash can expand and fuse into a larger moss-like lesion if the disease lasts for a long time. The condition of neurodermatitis can be perennial or recurrent. In a few patients, the lesions may become infected secondary to excessive scratching or hot water immersion, and the local skin may become swollen or painful.  Treatment: First of all, attention should be paid to avoid scratching and physical (needling, hot water scalding, etc.) stimulation of the affected area, pay attention to rest, it is recommended to go to sleep before 11:00 at night; diet must reduce spicy food and do not drink alcohol, reasonable use of anti-itch agents such as: medium and long-acting glucocorticoid ointment, patients with more serious conditions can also take oral antihistamines with anti-itch effect (see my writing for details) H1-blockers in my article “Diagnosis and treatment of urticaria”). In some patients with stubborn disease, oral antihistamines should be taken for a longer period of time, and in those with significant mossy skin, glucocorticoid-based topical medications can be given to seal the package, and microdermabrasion and ultrasound drug infiltration can also be used.
