Items reflecting liver fibrosis and cirrhosis

  These include albumin (Alb), total bilirubin (TBil), monoamine oxidase (MAO), and serum protein electrophoresis. When a patient has liver fibrosis or cirrhosis, there is a decrease in serum albumin and total bilirubin along with an increase in monoamine oxidase. The degree of elevated gamma globulin in serum protein electrophoresis can evaluate the evolution and prognosis of chronic liver disease, suggesting hypocellularity and inability to remove endogenous or enteric-derived antigenic substances from the blood circulation.  In addition, more clinically used in recent years are hyaluronic acid (HA), laminin (LN), pre-collagen type III peptide and collagen type IV. Measurement of their serum levels can reflect changes in liver endothelial cells, lipid storage cells and fibroblasts, and if their serum levels are elevated it often suggests that patients may have liver fibrosis and cirrhosis.  1, PCIII (pre-collagen type III): reflecting type III collagen synthesis in the liver, the serum level is consistent with the degree of liver fibrillation and is significantly correlated with serum gamma globulin level. Normal value <120ug/L. 2, IV-C (type IV collagen): The main component of the basement membrane, reflecting the renewal rate of basement membrane collagen, the increased content can reflect the liver fibrillation process more sensitive, is one of the early signs of liver fibrillation. Normal value <75ug/L. 3. LN (laminin): A non-collagenous structural protein unique to the basement membrane, positively correlated with the degree of liver fibrosis activity and portal vein pressure. Normal value <130ug/ml. 4.HA (hyaluronidase): It is one of the matrix components, synthesized by interstitial cells, and can reflect the amount of fibers generated in the liver and the damaged state of hepatocytes more accurately and sensitively, and it is believed that this index can reflect the whole picture of the diseased liver more completely than liver biopsy. Normal value <110mg/L. 5.PLD (prolaminase): It is the key enzyme of collagen degradation, and is consistent with the active degree of collagen fiber proliferation in the liver, which is a good indicator of liver fiber progress. The normal value is 1107±19.5u/L [5][6].  Among the above mentioned serum liver fibrosis indicators, the common combination that has reached clinical consensus is PCIII+PCIV+HA+LN, which is called the four liver fibrosis tests.
