Can Gum Fistula Heal On Its Own?

Gingival fistulas usually do not heal on their own, and are treated with root canal therapy for the root canal that causes the fistula, as well as medication for the fistula, as described below. Gingival fistulas are usually caused by chronic periapical inflammation. The treatment begins with pulp opening and root canal preparation, in which the root canal causing the fistula should be prepared beyond the apical foramen. Afterwards, the inner wall of the fistula is scratched with a small spatula until it reaches the apical area, and the fistula is rinsed repeatedly with hydrogen peroxide, metronidazole, and saline, and a white foamy liquid may overflow from the fistula. Iodoform preparation and cotton twists containing iodoform are placed in the root canal and the canal is temporarily sealed to achieve fistula closure.
