Asthma is a lung disorder characterized by episodes of shortness of breath and prolonged expiration that is more common in children. Because the symptoms of the disease during acute attacks seriously affect the normal life and learning of children, it is of great concern to doctors and patients. In recent years, the author has adopted the methods of de-evilizing evil, resolving phlegm, clearing internal organs, removing blood stasis, and replenishing deficiency, and combined with clinical diagnosis and typing to treat the disease with good results, and the experience is described as follows:
1.Evil offending the lung guard type
The child often manifests as vicious chills and fever, sweating or no sweating, runny nose, coughing and asthma, watery chicken sound in the throat, thin white and greasy tongue coating, and floating and slippery pulse. This is due to the feeling of external evil, which makes the lung lose its propagation and lowering, and phlegm blocking the airway. The treatment is to remove the evil and expel the phlegm from the lung. And according to the different nature of feeling wind-cold and wind-heat, and the two methods of warming and clearing respectively. Wind-cold asthma with vicious cold and no sweat, it is appropriate to use the warm propagation method; wind-heat asthma with fever and sweat, then use the clear propagation method, accompanied by products to resolve phlegm. The lung will be declared when the evil is gone, and the asthma will be calmed when the phlegm is dissolved.
1.1 Basic formula
Raw Ephedra 3g, Almond 6g, Qian Hu 4g, Radix Panax notoginseng 6g, Pericarpium Citri Reticulatae 4g, Radix et Rhizoma Polygonatum (packet), Su Zi, White Mustard Seed, Lycopodium each 5g, Rhizoma Polygonatum 5g, Licorice 3. If the wind is cold, add cinnamon stick 5g; if the wind is hot, remove ephedra, add mulberry leaf and burdock 6g each, and dabai mum 10g; if fever is seen, remove suzi, white mustard seeds, and half ansia, add raw gypsum 15g, dilong 6g, scutellaria 5g, and change raw ephedra to roasted ephedra; if wheezing subsides, but still have cough, remove suzi, white mustard seeds, spineflower, and lycopodium, add poria 10g, winter flowers, purple Each 6g of Aster is used to improve the subsequent.
1.2 Case
Wu, male, 3 years old. The disease started in two days with chills and fever, no sweating, runny nose, followed by coughing and asthma, croup in the throat, and a thin white, slightly greasy tongue coating. The evidence belongs to wind-cold external bundle, loss of lung qi promotion, the treatment should be pungent and warm to relieve symptoms, promote lung and resolve phlegm. The prescription is 1.5g of raw ephedra, 3g of almonds and Qian Hu, 5g of orris, 5g of sage and peppermint, 5g of Phlegmia, 5g of Pericarpium, 3g of Radix et Rhizoma, 3g of licorice, and 1 slice of ginger. After two doses of medicine, the chills and fever had subsided and the asthma had subsided, but there was still a cough. The original formula removed Su Ye, Peppermint and Ginger, and added Poria, Winter Flower and Aster 6g each, which was cured with two doses.
2.Phlegm and turbidity internal obstruction type
Most of the manifestations are Qi rebellion and wheezing, phlegm and saliva upward like tide, with thick and greasy tongue coating and sunken and slippery pulse. This type is free of superficial evil and can be easily distinguished from those induced by external sensation. The main rule of treatment is to clear phlegm and turbidity. However, in the identification fashion, attention must be paid to the characteristics of the internal organs and the same source of phlegm and stasis in order to receive satisfactory results. If accompanied by hot flushes, abdominal distension and knotting, the asthma is mostly due to the stagnation of the intestines and stomach, the internal Qi does not work because the lung and the large intestine, the purging of the lung Qi depends on the smooth flow of the internal Qi, if the large intestine stagnation does not work, it can affect the purging of the lung Qi, phlegm Yong will be knotted and affect each other, resulting in phlegm and turbidity blocking the airway. Therefore, the treatment does not only focus on the purging of the lung and lowering the rebellion, but also pay special attention to the clearing of the bowel. If the asthma attack is accompanied by blue face, blue lips and purple tongue, the asthma is not caused by phlegm, but by stasis of the ligaments, which is formed by “phlegm plus stasis of blood”, the treatment should pay attention to the use of blood activation and blood stasis.
2.1 Basic formula
3g of Bile South Star, 6g of Citrus Aurantium, 6g of Scapularia, 5g of each of Su Zi, Mustard Seed and Lycopodium, 6g of Radix Panax notoginseng, 3g of Chen Pi, 10g of Poria, 6g of Almond, 6g of Salvia, 2g of Glycyrrhiza glabra. 6g of Raw Rhubarb (later), 12g of Raw Gypsum (first decoction), 10g of Guadua Ren, blue face, blue lips, purple tongue or stasis spots on the side. Remove soursop, mustard seeds and lycopodium; add 5g each of peach kernel, safflower and tulip; take 10ml of fresh bamboo leek for those with phlegm and saliva.
2.2 Case
Liu, female, 8 years old, has suffered from asthma for more than 3 years. This attack was caused by a poor diet and a recurrence of external evil for 1 week. She has a cough with rebellious air, wheezing and phlegm, sounding like a saw, vomiting mucous phlegm, abdominal distention and fullness, and heat in the afternoon. The stool has not been relieved for two days, the urine is yellow and little, and the tongue is red with a yellow, greasy coating. This is a case of phlegm-heat blocking the lung and heat knotting the stomach and internal organs. The treatment should be to pass the internal organs and lower the rebellion, diarrhea and calm the asthma. The prescription is 6g of raw rhubarb, 15g of raw gypsum, 6g of almonds, 10g of guajouren, 6g each of scape hebecarpa seeds, suzi, lycopodium, spineflower, roasted ephedra and heliotrope shell, 10g of jiao hawthorn, and 1.5g of licorice. The disease was cured after three days of treatment with the regimen.
2.3 Case
Li, male, three years old. He started to get asthma one year ago, and his asthma is bound to come on every time he gets cold, and he can have attacks in all seasons. This time, the attack has been 4 days, and he came to the clinic after being treated locally and using Chinese and Western medicines to no avail. He has a cough and wheezing, a roaring in the throat, difficulty in lying down when holding his breath, a blue face and lips when wheezing, a purple and dark tongue, a white, greasy and moist coating, and a phlegm and stagnant blood stasis. Prescription: 5g each of Su Zi, Mustard Seed, Lycopodium, Di Long, Yu Jin, Tao Ren, Safflower and Almond, 3g each of Chen Pi, 5g of Radix et Rhizoma Polygoni, 3g of Roasted Ephedra, 2g of Licorice and 2g of Cinnamomum, two doses.
After the medicine, the asthma and phlegm were significantly reduced, and the cyanosis also disappeared, followed by the original formula with reduction and treatment for more than a week.
3.Virtually mixed type
The symptoms include muscle relaxation, mental discomfort, coughing and wheezing, especially when moving, coughing in the throat, white or yellowish face, light tongue with greasy coating, thin and feeble pulse, etc. This is not only a symptom of external evil visiting the lung, but also a symptom of deficiency of the lung, spleen and kidney. The author believes that this type is mostly seen in children with congenital deficiency or physical weakness, and the treatment is based on the principle of “support the righteousness before the onset of the disease, and attack the evil as a matter of urgency after the onset of the disease”. In addition, it is necessary to distinguish between heat and cold as well as yin and yang bias, and add and subtract treatment according to the evidence in order to calm the asthma.
3.1 Basic formula
For yang deficiency, add 5g of Radix et Rhizoma, 3g of Radix et Rhizoma; for yin deficiency, add 10g of Radix et Rhizoma, 5g of Radix et Rhizoma; for blue lips and dark tongue, add 5g of Radix et Rhizoma; for fever, add 10g of Radix et Rhizoma; for constipation, add 10g of Radix et Rhizoma; for fever, add 10g of Radix et Rhizoma; for constipation, add 5g of Radix et Rhizoma. For feverish constipation, add raw rhubarb (posteriorly) 5g; for hot phlegm, add guahua 10g, chuanbei 6g, dilong 5g; for cold phlegm, add bile south star 2g, mustard seed 5g.
3.2 Case
Wang, female, 10 years old. She has been suffering from recurrent asthma attacks for 8 years with one week exacerbation. Diagnosis: cough and asthma, wheezing when moving, inability to lie down, white sputum, mental discomfort, chest tightness and shortness of breath, lack of appetite, normal second stomach, pale tongue with white coating and moistening, sunken and thin pulse. The evidence belongs to the deficiency of the lung, spleen and kidney endowment, long-standing illness is deficient, when the spleen to strengthen the kidney to promote the lung and resolve phlegm. Prescription: 10g of Yam, 6g of Radix Rehmanniae, 10g each of Walnut and Astragalus, 6g each of Roasted Ephedra, Almond, Su Zi, Fa Han Xia, Wu Wei Zi, Poria, 3g of Chen Pi, 2g of Licorice. five doses, and 20g of Zi He Che powder, 2g per dose, twice daily.
After the medicine, asthma was significantly alleviated, the spirit was greatly improved, and the diet was enhanced. He was advised to continue to take 10 doses of the original formula and 1 dose every other day for consolidation.
DISCUSSION
The onset of asthma is mostly due to the presence of internal evil (the three reservoirs of the lungs, spleen and kidneys are deficient in phlegm and saliva), which is triggered by external evil (climate change, cold and temperature disorders, contact with foreign bodies, excessive consumption of raw, cold, salty and sour things, etc.). I think that the combination of treatment by removing the evil, resolving phlegm, clearing the internal organs, eliminating stasis and replenishing deficiency is more effective than simply choosing one treatment method. In addition, the long-term disease must take into account the spleen and kidney, without leaving the evil in the premise of supporting the root can play a more effective role.
