How should I choose the procedure for labiaplasty?

In clinical practice, there are many women who struggle with what kind of procedure they should choose for labia minora surgery before surgery. In fact, it is not necessary, there are many methods of labia minora plastic surgery, such as edge excision method, wedge excision method, lower pole method, full layer posterior pole method and so on. The actual fact is that some patients are not able to be solved in one way, but need to be removed by a combination of means according to the patient’s own characteristics. An experienced surgeon will fully communicate with the patient according to the specific situation and desired shape of the patient, so as to adopt the appropriate method and individualize the treatment. The basic procedures of common labia minora surgery are labia minora edge excision suture, wedge excision and central de-epithelialization suture. The rest of the procedures are mostly modifications and combinations of the above basic procedures. To perform labia minora surgery, the following principles should be followed: 1) to maintain the natural shape and functional integrity of labia minora after plastic surgery; 2) to avoid excessive surgical trauma, minimally invasive, safe and with few complications; 3) to hide and reduce scar tissue as much as possible to achieve a natural and beautiful appearance of labia minora. Labia minora edge excision suture method: Labia minora linear excision suture method is one of the earliest methods used in labia minora reduction surgery and is still widely used. It is easy to operate and can remove the dark brown edge while removing the hypertrophied labia minora to form a new lighter colored edge, which is visually more youthful and suitable for women with overly hypertrophied labia minora and who attach importance to the postoperative labia minora shape. However, linear labia minora excision and suturing also has its defects: it may cause nerve damage and postoperative labia minora sensory loss. Wedge-shaped labia minora excision method: Wedge-shaped excision This method preserves the natural shape of the edges of the labia minora and removes the most hypertrophic part of the tissue in the middle part of the labia minora, and is suitable for patients with excessive tissue in the middle elongation and long axis direction of the tongue-like labia minora. The procedure has a shorter scar and reduces nerve damage. The disadvantage is that the inconsistent length of the broken ends on both sides of the labia minora can lead to poor anastomosis, and the color jump on both sides of the incision is more obvious and affects the aesthetics. The central de-epithelialization suture method: The central de-epithelialization suture method is to remove only the inner and outer epithelial tissues of the labia minora, thus avoiding nerve damage, preserving the original contour of the labia minora to the greatest extent, reducing postoperative irritation and painful sex life, and achieving natural cosmetic effects, but there may be cases where the labia minora appears to be thicker than before surgery or folds may appear. However, this method cannot remove the overly long labia minora edges and is only suitable for patients with mildly enlarged and thin labia minora. There is no one labiaplasty procedure that has all the advantages over the others. The surgeon needs to master as many surgical approaches as possible and choose the appropriate surgical option based on the patient’s labia minora shape and the patient’s aesthetic appeal. If the patient needs to preserve the natural appearance of the labia minora, a wedge-shaped excision can be performed; if the patient needs a rejuvenated appearance of the labia minora after surgery, a linear excision and suture of the labia minora can be performed; if the labia minora protrudes less than 2 cm from the labia majora, a central debridement and suture of the labia minora can be performed; if the labia minora protrudes more than 2 cm from the labia majora, a full excision of the labia minora can be performed. According to the size of the protruding labia minora and whether the patient needs to retain the natural color of the original labia minora and other aesthetic demands, the appropriate surgical method can be selected to maximize the satisfaction of the patient.
