In addition to chronic nervousness and anxiety, acute panic attacks, the patient also had somatic manifestations of anxiety – finger twitching, consistent with the diagnosis of anxiety disorder, treated with the anti-anxiety drug paroxetine, I told the patient that this is currently the most common and effective drug for the treatment of anxiety disorders. The patient accepted my statement and agreed to take the medication on a regular basis.  At the follow-up appointment 1 month later, the patient’s anxiety and somatic anxiety symptom clusters had largely disappeared, his Hamilton Anxiety Inventory score was ≤7, and he was clinically cured in the acute stage. He said, “I used to massage my neck every week, but since I took paroxetine, my neck is no longer stiff and sore, and I dare to go out alone, and I have no problem with my basic life and work. Even if there are some troubles in my life, I can deal with them openly and let nature take its course, so I no longer feel anxious all day long as if the world is ending. His wife is also happy to say that she is much better now and basically doesn’t give me a tantrum anymore, and has not had a single “heart attack”.  This is a typical case of anxiety disorder. The stiffness and pain in the neck muscles are actually physical symptoms of anxiety, just like the chest tightness, shortness of breath, dyspnea, and throat blockage symptoms of the patient, and the nervousness is an emotional symptom of anxiety. It is a pathological anxiety because it is spontaneous, persistent, painful, and interferes with daily functioning, leading to abnormal behaviors in patients such as help-seeking and avoidance behaviors. Treatment for anxiety disorders is medication + psychotherapy, which provides effective relief in 80% of patients.  The patient in this case responded well to medication, was compliant, and was able to return to the clinic on time. The complete treatment for him was: 2 months for the acute phase, 6 months for the recovery phase, and 6-9 months for the maintenance phase.  After 10 months, the patient followed up again and reported that everything was normal. He claimed that he also climbed Mount Hua in September, saying that he had always wanted to visit Mount Hua, but was afraid to go because of his anxiety. This time, there was no fear, discomfort or anxiety in climbing Mount Hua, and felt that Mount Hua was not as treacherous as imagined or legendary, thankfully.  Anxiety disorder belongs to chronic episodic neurological disorder, it is a treatable and preventable disease, it is the first common mental disorder, but the recognition rate of anxiety disorder by physicians in other departments is low, improve the awareness of anxiety disorder by physicians in various departments, so that patients with anxiety disorder can get specialist consultation and treatment, so that anxiety and anxiety no longer bother patients and families.
