I. Early postoperative functional exercise: 1. Passive exercise: (6 hours-2 weeks postoperatively) Methods: 1. 6 hours postoperatively, operated by the accompanying person, massage the posterior and lateral muscles of the calf from far to near, 15-20 minutes/times, 4-6 times/day. 2.Performed 6 hours after surgery, operated by the chaperone, one hand slightly flexes the knee joint to about 10°, one hand holds the plantar ankle, and then is flexed the ankle joint to about 20°, maintained for 1-2 seconds, 15-20 times/group, 4-6 groups/day. 2.Active exercise: (2 to 12 weeks postoperatively) (1) Ankle pump motor function exercise: performed on the 2nd day postoperatively. Methods: with the knee in the straight position and the anterior thigh taut, actively dorsiflex the ankle joint to the limit and make the movement of hooking the foot, then actively dorsiflex the ankle joint downward to the limit and make the movement of sitting down and stepping down. Each maintain 3-5 seconds, 10-15 times/group, 4-6 groups/day. (2) Quadriceps power diastolic functional exercise (isotonic contraction): performed on the 2nd postoperative day. Method A: slightly flex the knee to about 20°, then slowly straighten the calf and hook the foot upward, keep it for about 5-10 seconds, then slowly lower it, and repeat, 10-15 times/group, 4-6 groups/day. Method B: sit on the edge of the bed, bend the knee joint, slowly lift up to stretch flat and hook the foot upward, maintain about 5-10 seconds, and then slowly put it down, so repeat, 10-15 times/group, 4-6 groups/day. (3) Gastrocnemius muscle contraction function exercise: performed on the 2nd postoperative day. Methods: with the knee joint in the straight position, take the heel as the fulcrum, take the initiative to slowly flex the knee to about 25°, do the action of grinding the ankle toward the proximal end of the body, and then take the initiative to slowly flatten the calf, and so on and so forth, repeat, 10 times/group, 4-6 groups/day. Second, postoperative functional exercise in the middle period: (from the 7th week to 6 months after surgery) Functional exercise of the gluteus medius muscle is the main focus, which starts from the 7th week after surgery. Lying on the healthy side, slowly elevate the affected side slightly backward by 15°, gradually to about 20°, and maintain for about 3-5 seconds, 10-15 times/group, 4-6 groups/day. Standing position, the healthy side lying position, the affected side of the extension position of slow abduction, do outward leg extension, progressive to about 15 ° – 20 °, and maintain about 3-5 seconds or so, 15-20 times / group, 4-6 groups / day. Third, postoperative life taboos: the affected side can be sagging after getting down to the ground, after adapting to the gradual weight-bearing, step by step, but 6 weeks must be walking with crutches, 1.5 months after the beginning of the abandonment of the crutches to walk (special circumstances depending on the follow up). 6 weeks of attention: sitting on the station to go to the toilet, turn over to pinch the pillow; taboos: flexion of the hip joint > 90 °, the affected limb is not inwardly retracted, do not rotate, and do not encourage the straight-legged elevation exercise. Forever contraindication after three months: weight-bearing more than 20 kilograms, long-distance walking (to their own fatigue) and so on. Review 6 weeks and 3 months after surgery, and follow up for life. Advice: postoperative anticoagulation for 1 month to prevent thrombosis.
