Cardia relaxation refers to the inability of the cardia to contract effectively due to inadequate function of the lower oesophageal sphincter and other muscles, while cardia inactivation is caused by increased internal pressure in the lower oesophagus due to multiple factors such as reduced oesophageal tone and peristalsis and the inability of the lower oesophageal sphincter to relax due to oesophageal neuropathy, which leads to oesophageal dilatation.
The distinction between achalasia and culpable flaccidity lies in the marked dilatation of the oesophagus in the former, with a ratty narrowing of the cardia and increased pressure in the cardia region.
