1.What is gluteus contracture? Gluteus contracture is a clinical syndrome caused by the degeneration and contracture of the gluteus muscle and its fascia fibers from various causes, resulting in the characteristic gait and signs of hip joint function limitation. Since Valderrama’s report in 1970, there have been many reports at home and abroad, but the etiology is not very clear, and it is mostly related to gluteal muscle injection in early childhood. 2, the clinical manifestations of hip contracture Clinical manifestations are mainly the hip joint popping and hip joint dysfunction. The patient’s hip joint internal rotation and internal retraction activities are limited. When standing, the lower limbs are externally rotated and cannot be fully brought together. When walking, there is often an outward eight, swaying gait, and a jumping state when walking fast. When sitting, the legs cannot come together, the hips are separated in frog position, and it is difficult to rest one thigh on the other thigh (cross-leg test). When squatting, the lighter person squats with both knees apart, then squats and then comes together (circle test). In heavy cases, the hip joints can only squat in the abducted and externally rotated position, and the knees cannot come together when squatting, and the heels do not touch the ground, showing a frog-like pattern. Physical examination reveals skin depression in the upper part of the hip, the lower limbs are in the abducted and externally rotated position, and the hip is limited in internal retraction and internal rotation. Ober’s sign is positive. In severe cases, there may be pelvic tilt and compensatory scoliosis of the spine. 3.Treatment of hip contracture Traditionally, the hip muscle contracture is released by incision, which has the disadvantages of large surgical trauma, bleeding, easy to damage the sciatic nerve and residual cavity after surgery. At present, our department can adopt arthroscopic release of gluteus contracture, with small surgical incision, small trauma, fast recovery and satisfactory results, and many foreign patients come to us for treatment.
