Gluteus contracture, also known as popping hip, is a group of clinical syndromes caused by degenerative contracture of the gluteal muscles and fascia fibers, which restricts the function of the hip joint, such as internal rotation, thus showing the characteristic gait and abnormal posture. The main symptom is external rotation of the lower limbs when standing, which cannot be completely close together. When sitting down, the legs cannot come together, the hips are separated in frog position, and it is difficult to rest one thigh on the other thigh; when squatting, the lighter person squats with the knees separated first, then squats and then comes together; the heavier person can only squat in the abducted and externally rotated position, and when squatting, the hips are in the abducted and externally rotated position, the knees cannot come together, and the heels do not touch the ground, in frog position. The causes of the disease can be divided into two aspects: injection factors, some literature reported that benzyl alcohol diluted penicillin can lead to irreversible damage to the gluteal muscle, the higher the frequency of intramuscular injection the greater the possibility of contracture, the dose of intramuscular injection is also an important risk factor; genetic factors, some literature through the study of 42 cases have 5 generations of relatives, siblings at the same time, 6 pairs of 12 people, that the disease and genetic congenital factors There is a greater relationship with genetic predisposition. For the treatment of gluteus contracture, it is generally believed that the disease should be operated as soon as possible after diagnosis to avoid osteoarthropathy. Experimental studies on simulated biomechanics of gluteus contracture were conducted, and the results found that the hip joint force, gluteus maximus, gluteus medius, and gluteus minimus force of patients with gluteus contracture decreased to different degrees, and the change in muscle strength led to an imbalance in pelvic mechanics, and the side of the iliac bone with less gluteus maximus atrophy was more negative, which could lead to local ischemia, hypoxia and sclerosis. Early treatment of patients can effectively reduce the likelihood of patients developing other secondary diseases. example, it is believed that the disease is more related to hereditary congenital factors. In the traditional open surgical treatment of gluteus contracture, the contracture bundle is partially excised and the iliac crest is dissected, and the outer plate of the skeleton is peeled off, so that the starting point of the gluteus muscle is shifted downward, or the diseased muscle is lengthened in a “z” shape, which has many disadvantages such as large incision, large bleeding, high risk of postoperative pain, and long surgical scar.
