–After pelvic radiotherapy for patients with pelvic malignant tumors, many of them have symptoms of mucous blood stool, urgency and heaviness, abdominal pain and falling down in varying degrees, which is called “radiological enteritis”. There are many reports in Chinese medicine clinic, the treatment effect is very different, the reason may be the “radioactive enteritis” disease in Chinese medicine disease category of understanding the differences. In our clinical practice, we grasp the high consistency between the main symptoms of this disease “mucous bloody stools, acute and severe abdominal pain and falling” and the main symptoms of “dysentery” in Chinese medicine, and use “dysentery” to identify and treat the disease. In this study, a total of 33 cases of radiation enteritis were treated from March 2007 to March 2009, and good therapeutic effect was achieved. Male 14 cases, female 19 cases. Received pelvic radiotherapy: 15MeVX ray, 50Gy/20～25 times, 28～32d consecutively. 1.2 Diagnostic criteria Ministry of Health, Department of Medical Affairs, edited by the Chinese Common Malignant Tumor Diagnosis and Treatment Code (1) history of pelvic radiotherapy; (2) intestinal symptoms: mucous blood stool, urgency and heaviness, abdominal pain and drop; (3) rectal fingerprinting: the tissue of the anterior wall of the rectum is stiff, or accompanied with varying degrees of rectal stenosis, and finger sleeve (4) Fiberoptic enteroscopy or proctoscopy: rectal mucosa is congested and edematous, brittle, with point-like or sheet-like bleeding, or with signs of tissue necrosis. 1.3 Efficacy assessment Cure: symptoms disappeared for more than 4 weeks, normal stools, negative occult blood, no mucus or blood on rectal palpation, rectal microscopy restored to normal or only intestinal mucosal texture is not clear; Benefit: Clinical symptoms improved or symptoms disappeared repeatedly after the disappearance of the disappearance of the bloody stools of the naked eye, occult blood positive, rectal microscopy can see the intestinal mucosa of a mild congestion and edema, a small amount of point-like bleeding; Ineffective: Clinical symptoms and proctoscopy have not been improved. Ineffective: no improvement in clinical symptoms and proctoscopy. 1.4 Treatment method and course 1.4.1 TCM diagnosis Yin deficiency and damp-heat type – Yin deficiency dysentery 28 cases, prescription: Huanglian, Agaricus, Angelica sinensis, red and white peony, Sha Shen, Dendrobium officinale, Dampyxis, Diyu charcoal, licorice, gunner’s ginger; Spleen deficiency and damp-heat type – resting dysentery 5 cases, prescription: Pueraria Mirifica, Radix et Rhizoma Ginseng, Rhizoma Atractylodis Macrocephalae, Radix Bupleurum Chinense, Radix Rehmanniae Praeparata, Radix Rehmanniae Praeparata, Radix Melliferae, Hawthorn, Radix Paeoniae Alba, Pericarpium Citri Reticulatae, Dried Ginger, Glycyrrhiza Uralensis. They were all taken orally with decoction of water. 1.4.2 Course of treatment The herbal medicines are decocted in water and taken internally, and one month is one course of treatment. 1.4.3 Follow-up 3 months after the end of treatment. Results: 27 cases (81.8%) were cured, 6 cases (18.2%) were improved, and 0 cases were invalid. 3, Discussion There are three major categories of diseases with increased stool frequency and loose feces as the main symptoms in TCM internal medicine: diarrhea, dysentery and cholera. In the Chinese medicine clinic, in addition to cholera, diarrhea and dysentery, these two types of diseases, including modern medicine, specific and non-specific enteritis disease, every encounter with intestinal inflammatory diseases, in the writing of the case is also accustomed to this type of disease is attributed to Chinese medicine diarrhea, dysentery, etc., the use of traditional Chinese medicine is often the use of traditional Chinese medicine with modern medical thinking, and the results are often unable to get a satisfactory outcome, or even because of the effect of traditional Chinese medicine is not good, and simply apply Western medicine, this phenomenon in many cases, the use of Western medicine, and the use of traditional Chinese medicine. The use of Western medicine, this phenomenon is not uncommon in many Chinese medicine hospitals, surgery or anorectal department. In fact, the reason why the Chinese medicine internal science should be divided into diarrhea, dysentery two diseases, is in accordance with the analysis of Chinese medicine disease identification, these two types of diseases in the etiology of the disease mechanism is fundamentally different. According to the thinking of Chinese medicine treatment of disease, the first is to identify the evidence (disease), and then is the identification of the treatment, only when we learn Chinese medicine too much emphasis on identifying the evidence and ignore the identification of the evidence (disease), and even up to the current report is often enthusiastic about the different diseases because of the same type of evidence and cured with the same prescription. However, we believe that the identification of evidence (disease) in TCM is as important as the diagnosis in Western medicine. If the identification of evidence (disease) is wrong, just like the diagnosis of Western medicine is wrong, even if a patient is cured by fluke, it will be by chance. In fact, the internal medicine science of Chinese medicine in each disease chapter have class evidence identification, here the evidence is what we usually say disease. Chinese medicine internal science in diarrhea, dysentery, respectively, two diseases have been detailed and have a brief description of their main symptoms and the main mechanism of disease. Diarrhea is to increase the number of bowel movements, fecal thinning as a characteristic, spleen deficiency and wet victory is an important factor leading to the occurrence of this evidence; dysentery is to abdominal pain, acute and severe, dysentery red white mucus as the main evidence, the cause of this disease with the feeling of the seasonal evils (dampness, heat, epidemic, cold and dampness) and dietary disorders related to the qi and blood with the seasonal evils and fighting knot, intestinal stagnation of the qi and lipid injury, stagnation of qi and blood. In our clinic, we found that the main symptoms of radiation enteritis and the main evidence of dysentery in traditional Chinese medicine are almost identical, we use the identification of dysentery typing, radiation enteritis, 84.8% of the patients with yin deficiency and dampness-heat type of dysentery, i.e., for dysentery in the yin deficiency dysentery; there are also about 15.2% of the patients with the spleen deficiency and dampness-heat type of dysentery, i.e., for dysentery in the resting dysentery. Due to the correct identification of the disease, objective typing, and the use of traditional Chinese medicine prescription Zhucha Pill and Lizhong Tang combined with diarrhea san plus reduction, we obtained a cure rate of 81,8% and a recovery rate of 18.2%. Of course, in the treatment of radiation enteritis, we found that the heavier the symptoms, the better the efficacy, the lighter the symptoms, but the worse the efficacy, which may be related to the Chinese medicine said that the disease is long and lingering, the complexity of the disease mechanism, the mixture of real and virtual, which is also the direction of the future in the clinic we need to continue to explore or whether to increase the intestinal beneficial bacterial flora in conjunction with modern medicine can make a breakthrough.
