Torn ACL is a common clinical injury. Many patients ask before undergoing surgery: Can I return to sports after ligament reconstruction? This is a difficult question to answer. There are many clinical uncertainties and it is difficult to generalize. 1. The time to receive the surgery is important! In general, it is recommended that early surgery be performed after the first injury if an ACL tear is diagnosed. Some literature suggests that 2 weeks after the first injury is the best time and that premature surgery may increase the likelihood of postoperative adhesions, but this point in time is not unanimously accepted. Most opinions in the literature suggest that surgery can be performed after knee mobility can reach 90 degrees of knee flexion and that the risk of postoperative adhesions is low. However, for various reasons such as lack of patient attention after injury, in-country visits and the need for MRI appointments, it is often more common to see subacute or old ACL ruptures of the knee more than 3 weeks after the injury, even in cases where the patient is seen after multiple sprains. According to the time between the first injury and the definitive diagnosis, these patients can be divided into 2 main categories: 1. cases within 6 months of the first injury: these cases do not necessarily present with secondary meniscal or cartilage damage and have a relatively good functional recovery after surgery, with a greater likelihood of returning to sports; 2. cases after 6 months of the first injury: these cases continue to walk or play sports after the ACL rupture, with progressive wear and tear on the joint. Most of these cases have secondary meniscal or cartilage injury, thigh muscle atrophy, and ACL ligament stumps have been absorbed, requiring longer postoperative rehabilitation to return to sports. Overall, the longer the ACL tear and the more severe the combined injury, the lower the chances of returning to sports after surgery. If certain important structures, such as a torn lateral meniscus, have to be completely cut during surgery due to severe wear, return to sports after surgery is essentially impossible and early degeneration of the joint can occur, affecting the quality of life later in life. Therefore, the earlier the surgery is performed after the diagnosis of ACL tear, the greater the possibility of returning to sports! 2. The surgery itself is important! There is no doubt that the ligament reconstruction surgery itself is a key factor in determining whether you can return to sports after the surgery. The surgical approach, the choice of ligament grafts, the fixation method, the experience of the surgeon, etc. are all very important factors. There are hundreds of academic papers each year on the topic of ACL reconstruction, but to date there is no universally accepted best surgical approach, and different surgeons have their own familiar and specialized surgical approaches, as well as preferred graft and fixation options. Based on my own experience in ACL reconstruction for more than 10 years and the current academic mainstream opinion, I recommend single bundle reconstruction of the ACL using the anatomical position of the autologous N cord tendon with Introfix extrusion screws on both the femoral and tibial sides to obtain effective and secure fixation with good immediate or long-term postoperative results and no residual metallic foreign bodies in the body after surgery and no need for secondary surgery to remove the internal The fixation does not require a second surgery. Of course, according to certain objective factors, such as age, their own will, economic conditions, etc., sometimes need to do some appropriate adjustments. 3. Self-care after surgery is very important! After the completion of ACL reconstruction surgery, there are several key issues that affect the efficacy of the surgery, such as the degree of wound healing, the degree of muscle atrophy, the degree of graft healing, etc. These require self-care and self-care, mainly including the following points: (1) reasonable diet: avoid tobacco, alcohol and allergy-causing foods, many studies have confirmed that smoking after surgery can cause poor wound healing. (2) Regular change of medication: generally about 3 days in the regular hospital for drug changes, if there is abnormal exudation, you need to promptly follow up with the surgeon; (3) regular work and rest, avoid fatigue: the trauma of ACL reconstruction surgery itself will cause the human immune system to decline, and on the basis of this irregular life (such as staying up late, etc.) and excessive fatigue can easily lead to bacterial or viral infections such as colds and sore throats, and enter the operated joint along the blood stream and lead to joint infection. This joint infection many times takes 1-2 months to gradually recover, thus affecting the recovery process. (4) Follow medical advice for rehabilitation: See Article 4 for details. 4. Post-surgical rehabilitation is important! After a successful surgery, in fact, the road to recovery is only 50% gone. We sometimes say: three points rely on surgery, seven points rely on rehabilitation. Although it is a bit too much, there is no doubt that the rehabilitation process is very important. However, unfortunately, because the domestic rehabilitation and physiotherapy system is not perfect, many times patients need to do rehabilitation training at home by themselves, resulting in many patients being too aggressive or too conservative to achieve the desired purpose of restoring movement. As a key specialty of the Ministry of Health and a leading unit of sports medicine in China, our department has set up rehabilitation doctors and full-time physiotherapists in the department to provide regular rehabilitation guidance to patients after ACL reconstruction, in addition to which we make a perfect rehabilitation plan according to the surgery.
