They suggested to establish and promote the application of individualized diagnosis and treatment programs based on clinical evidence for fatty liver disease combination, starting from solving the key clinical problems of fatty liver. “This is an important element in building a national system for the prevention and treatment of chronic diseases, as well as an important effort to improve the clinical capacity and level of Chinese medicine in the treatment of common and multiple diseases.” As a component of metabolic diseases, fatty liver has become the most prevalent liver disease in the 21st century. Studies have shown that the prevalence of fatty liver in developed cities in China has reached about 15%, making it a major public health problem in China. According to Xu Jianguang, president of Shanghai University of Traditional Chinese Medicine, “early intervention of fatty liver is an important path to reduce the risk of metabolic syndrome and cardiovascular and cerebrovascular events.” At the same time, he believes that lifestyle interventions have a better effect on early and mild fatty liver, but drug interventions are needed for fatty liver with abnormal liver enzymes and with components related to metabolic syndrome. Experts at the conference pointed out that Western medicine interventions for fatty liver and related complications, such as diet control, weight reduction, blood sugar and blood pressure control, and correction of disorders of glucolipid metabolism, can effectively control the progress of fatty liver disease, but there are disadvantages such as taking a wide variety of drugs, side effects and expensive. The hepatotoxicity of the commonly used statin lipid-lowering drugs limits their application for a long course of treatment, and the actual improvement effect on fatty liver needs to be further clarified; insulin sensitizers are recommended for the treatment of fatty liver with insulin resistance, but the disadvantage of not being able to effectively reduce body weight and the bladder lesions that have been reported recently will greatly limit their application in the treatment of fatty liver. Prof. Ji Guang, chairman of the conference and vice president of Shanghai University of Traditional Chinese Medicine, pointed out that the treatment of fatty liver in TCM is characterized by the importance of individual differences of patients, the combination of evidence and disease identification, and the adoption of comprehensive treatment measures, i.e., the use of pharmacological therapies (mainly herbal compounding) and non-pharmacological therapies together to enhance the efficacy and reduce side effects, thus improving the quality of life of patients and prolonging their lifespan. The multi-linked and multi-targeted nature of TCM makes it a promising treatment for fatty liver,” he said. Data show that over the past 30 years, TCM has been used to treat fatty liver mostly from spleen deficiency, phlegm-dampness and blood stasis, and has achieved certain efficacy, making TCM an important option for fatty liver treatment and an integral part of a comprehensive treatment plan. However, some experts say that the existing research model has not broken through the bottleneck of “efficacy improvement”.
