Among hyperthyroidism patients, about 13%-45% of them have invasive proptosis, which not only affects the appearance of patients, but also seriously affects their life, so it is necessary to take effective treatment measures. At present, there are many methods to treat hyperthyroidism with invasive proptosis, but most of them are ineffective and have large toxic side effects, so they cannot be widely used in clinical practice. After years of clinical observation and some literature reports, the effect of Yunque in the treatment of proptosis is good and worthy of clinical promotion. For patients concerned about the mechanism of action, the method of use and the judgment of efficacy and other hot issues to make a brief introduction. 1.The mechanism of action of Yunque: Yunque uses 99Tc to scavenge free radicals in the body through valence changes, protect the vitality of superoxide dismutase (SOD), and prevent the damage of free radicals on the human body. It inhibits the formation of immune complexes and regulates human immune function, especially it can significantly reduce the level of thyrotropin (TSH) receptor antibody (TRAb), thyroid microsomal antibody (MCA) and thyroglobulin antibody (TGA) to prevent the production of antigen-antibody complexes, thus reducing cell regeneration and glucosaminoglucan synthesis, preventing intraorbital collagen aggregation, preventing the increase of connective tissue and eye muscle hypertrophy, reduce orbital contents, and make the signs and symptoms of hyperthyroidism with malignant proptosis disappear, thus achieving the purpose of treatment [1]. 2. Usage of Yunker A+B: Before treatment with Yunker A+B, 5 ml of agent A is injected into the bottle of agent B under aseptic operation conditions, shaken sufficiently to dissolve the lyophilized material, and left to stand at room temperature for 5 minutes to obtain technetium [99Tc] methylene diphosphonate injection. Inject intravenously, 10mg/day, change to 5mg/day after 10 days, 20 days as a course, use one to three courses of treatment. The speed should be slow when pushing. Observation of efficacy: ①effective: the symptoms of eye swelling, foreign body sensation, photophobia, tearing and burning pain basically disappeared, periorbital edema and conjunctival congestion and edema significantly decreased, eyelid ptosis and ocular motility disorders basically returned to normal, and proptosis reduced by ≥2mm. ②Effective: The ocular symptoms, eye signs and proptosis detection were improved after treatment but did not reach the above criteria. (③Invalid: The eye symptoms, eye signs and proptosis test did not improve significantly or even worsened before and after treatment.
