Men are an important part of society and family, and play a pivotal role in the progress of society, but the pressure from work, family, society and other aspects make men encounter unprecedented “sexual” challenges. For example, erectile dysfunction is one of the most common male diseases and an important issue in modern male science. The treatment of erectile dysfunction (ED) varies greatly depending on the cause. After a detailed evaluation of patients with ED, treatment is usually based on pathogenesis. For most patients, a detailed history, physical examination and basic laboratory tests are sufficient, but for some special patients, further targeted invasive tests need to be used. The following will be briefly introduced from five aspects of my treatment of ED a little experience. First, non-pharmacological treatment: a portion of patients using non-pharmacological treatment can be relieved, lifestyle changes can be applied to change some of the risk factors for ED. Obesity and lack of physical activity are thought to be closely related to the occurrence of ED, with the higher the body mass index and sedentary lifestyle of the population the greater the likelihood of ED. Meanwhile, smoking, high blood pressure and diabetes all increase the risk of ED. A randomized trial has confirmed that weight loss and increased physical activity can significantly improve penile erectile function. Second, psychosexual counseling: has been successful in different types of ED patients. Usually, psychological counseling is most effective in patients with psychologically caused ED. These patients may be physiologically normal and just have some cognitive deficiencies. Third, vacuum negative pressure erection device: is a non-pharmacological treatment aimed at physically causing an erection rather than changing risk factors. Advantages of this device include the ability to produce an erection hard enough for sexual intercourse, filling the head of the penis with blood and making it larger. Disadvantages are that it can produce pain at the site of the ring, reduced or non-ejaculation and penile sassafras. Fourth, the drug treatment of ED: the most commonly used and more effective drug for the treatment of ED is phosphodiesterase type 5 (PDE-5), PDE-5 is by strengthening the efficacy of NO in the cavernous smooth muscle to play a role. NO stimulates guanylate cyclase leading to increased cGMP levels, and then these second messengers to reduce the level of intracellular calcium ions, which leads to smooth muscle relaxation and penile erection. PDE-5 inhibitors primarily block the conversion of cGMP to GMP and, therefore, inhibit phosphodiesterase thereby increasing the concentration of cGMP. At the same time, combined with testosterone replacement therapy and traditional Chinese medicine, you can receive the desired therapeutic effect. Fifth, the surgical treatment of ED: it is prepared for the patients who failed drug treatment or existed other reasons can not tolerate drug treatment. Surgical treatment is mainly penile prosthesis implantation. At present, the most used and most effective prosthesis in the clinic is the three-piece set. Penile revascularization may be performed in some patients with arterial ED due to pelvic injury. In short, as a medical worker, the most important thing is to give the patient psychological guidance, and sexual life guidance, so that they correctly understand the disease, establish self-confidence, and then according to the cause of the disease, orderly and favorable standardized treatment, and then combined with traditional Chinese medicine can have a satisfactory therapeutic results. And not in order to let the patient to cooperate with the doctor, exaggerate the severity of the disease, aggravate the psychological burden of the patient.
