Multiple sclerosis (MS) is an autoimmune disease characterized by inflammatory demyelination of the white matter of the central nervous system. The disease mostly starts at the age of 20-40; the main clinical manifestations are limb weakness, unstable walking, or even paralysis, as well as numbness, pain in the limbs, a feeling of lumbar back binding, and blurred vision. However, according to its clinical manifestations, it can be attributed to different TCM illnesses, such as: limb paralysis or weakness is mainly attributed to “impotence” in TCM; vision loss, diplopia or blindness is mainly attributed to “dimness of vision “The main cause of this disease is the “impotence” in Chinese medicine. According to the clinical characteristics of this disease, combined with the basic theory of Chinese medicine, we believe that the formation of this disease is mainly due to the deficiency of congenital endowment, repeated exposure to external evil, unfavorable qi-transformation, the production of turbidity and toxicity, damage to the governor and ligament, killing the kidney yang and brain marrow, resulting in the corruption of the body. The basic pathogenesis of the disease is that the deficiency of kidney yang is the root cause, and the internal accumulation of turbid toxins is the target. Therefore, the main treatment principle for this disease is to “benefit the kidney, resolve the turbidity, detoxify the toxins, and open the ligaments”. In clinical practice, multiple sclerosis often alternates between acute and remission phases, and different phases of the disease have corresponding TCM syndromes. Therefore, we adopt staged and evidence-based treatment, i.e., treating from the root (benefitting the kidney), removing pathological factors (detoxifying and resolving turbidity), and alleviating pathological damage (unblocking the ligaments). During the acute attack period, toxicity and turbidity are rampant, the disease is dangerous, and there are many symptoms, the evil is predominant during this period, damp-heat toxicity is common, turbidity and toxicity damage the Governor’s Vessel, and the disease affects kidney yang and brain marrow, leading to neurological dysfunction. Treatment is based on detoxification and elimination of turbidity, as well as benefiting the kidneys and opening the ligaments. During the remission period, the evil energy declines and the positive energy is also injured. During this period, the deficiency is mixed with the real, the Governor’s vein is not full, the spleen and kidney are deficient, and the brain marrow is not nourished, resulting in symptoms that are lingering and difficult to cure. Treatment is based on benefiting the kidney, resolving turbidity, and clearing the ligaments. In the treatment of multiple sclerosis, attention should be paid to individualized evidence-based treatment under the guidance of the holistic view of Chinese medicine, that is, for different disease stages and different symptoms, individualized treatment plans should be formulated in order to achieve the purpose of reducing symptoms, controlling the progress of the disease and preventing relapse.
