What is “celiac disease”?

  In clinical work, many female patients have a deep misunderstanding of “cervical erosion”, thinking that “cervical erosion” means “cervical rot”, and even associated with cervical cancer, causing unnecessary panic. This has caused unnecessary panic. Therefore, it is necessary to have a new understanding of the so-called “celiac disease”.  Cervical erosion is a misnomer that refers to the appearance of the cervical canal mucosa or mucus-secreting epithelium in the vaginal part of the cervix. True celiac disease refers to the loss, absence, and ulceration of the squamous epithelium with exposure of the epithelial stroma and is rarely seen clinically. Traditional meaning of cervical erosion: the red, finely granular appearance of the vaginal part of the cervix at the ectocervix is called cervical erosion. The erosion surface is covered by a single layer of columnar epithelium of the intact cervical canal, which is red due to the thinness of the columnar epithelium and the permeability of the underlying mesenchyme.  It is also important to recognize the meaning of the following terms so that all changes that differ from the normal cervix are not diagnosed as “cervical erosion” to reduce the risk of under-treatment and even under-diagnosis of cervical cancer. Ectocervical ectropion: the lining of the cervical canal protrudes outwards towards the cervix and is visible as a longitudinal fold, which can be congenital or due to injury. Acute cervicitis: microscopically, blood vessels are seen to be congested, mucosa and submucosal tissues, large amount of neutrophil infiltration around the glands, and purulent secretions are seen in the glandular cavity, requiring treatment. Chronic cervicitis: microscopically, a moderate amount of lymphocytic infiltration of the mucosa and submucosal tissues is seen, and squamous metaplasia of the overlying epithelium may occur, with no obvious clinical significance.  A concept should be established that “cervical erosion” is erosion without epithelium, but only the covered columnar epithelium, which is displaced by the squamous-columnar junction during a woman’s lifetime due to changes in estrogen levels, in a sense “cervical erosion” is a young woman’s The “capital”. The term celiac disease should be gradually abandoned. Ectopic cervical columnar epithelium: A single layer of columnar epithelium of the cervical canal is exposed in the vaginal part of the cervix. It is a physiological change that reflects the estrogen level in the body. However, in normal clinical practice, the columnar epithelium is difficult to observe with the naked eye using a dilator alone and can only be observed under colposcopy with 3%-5% acetic acid solution. Do not interfere too much with the normal physiological state.  The prerequisites for intervention are: 1) obvious clinical symptoms; 2) exclusion of cervical malignant lesions, which can be confirmed by cervical cancer screening, such as cervical cytology, high-risk HPV DNA testing and colposcopy, to confirm that there is no possibility of malignant changes before intervention, which is preferable to physical methods; 3) treatment of specific pathogenic infections; 4) informed consent of the patient; 5) reduction of overtreatment.  Therefore, while doing a good job of cervical cancer screening, do not interfere too much with the real physiological cervical erosion, especially those young lesbians who have not had children and do not have clinical symptoms, a famous domestic cervical expert once made this analogy: “Cervical erosion” is like a small pearl growing on the cervix of young women, and so on.
