Microwave ablation of metastatic lymph node cancer in the neck

On April 25, 2015, Shanghai Zhabei District Central Hospital successfully performed ultrasound-guided microwave fractionated ablation of percutaneous cervical lymph node metastasis for an 86-year-old male patient with metastatic cancer in the cervical lymph nodes. Before the operation, considering the patient’s advanced age, metastatic thyroid cancer, huge lesions and proximity to large blood vessels, the risk of re-anesthesia and surgery was huge, and many hospitals refused to admit him. Fortunately, due to the adequate and perfect preoperative planning of Zhabei District Central Hospital, the operation was finally done successfully, with no damage to blood vessels and nerves, and the tumor was also inactivated twice with special precision. This technology is the first hospital in Zhabei District of Shanghai to successfully carry out this procedure, marking another innovation and enhancement of microwave ablation therapy in Zhabei District Central Hospital, which will continue to explore new peaks in the medical field and protect the health of the people. Performing surgery for patients In order to broaden the means of diagnosis and treatment for patients and reduce their pain, Zhabei District Central Hospital introduced in 2014 an original new technology and new treatment with independent intellectual property rights – ultrasound-guided microwave ablation of percutaneous thyroid nodules. The founder of this technology, Professor Zhang Jianquan, director of the ultrasound department of Shanghai Long March Hospital, personally instructed and taught the treatment technology, and the hospital was led by the medical department to form an interventional ultrasound collaboration group consisting of the Department of Physical Medicine, the Department of Anesthesiology, the Department of Endocrinology, the Department of Urology and the Department of Nephrology. The first microwave ablation of thyroid nodules was successfully performed on August 22 last year. As of May 20, 77 cases of microwave ablation for benign and malignant thyroid nodules, parathyroid hyperplasia, hepatic hemangioma, kidney cancer and other diseases have been successfully performed, and the procedure was performed under local anesthesia and took only half an hour, bringing good news to patients. Thyroid nodules are a common disease, and western medicine can only control the development of the thyroid gland, and the course of medication is long, which is harmful to the liver and kidney function. If the nodules continue to grow, the traditional treatment for thyroid nodule disease is open surgical excision. However, surgical removal of the thyroid lesion is very traumatic, often damaging the normal gland and causing complications, and the thyroid gland is damaged or removed, resulting in decreased function and the need for lifelong medication to control thyroid function levels. Moreover, it is difficult to guarantee that patients will not have a recurrence after surgery, and it is extremely difficult to operate again after a recurrence. In addition, the surgery leaves a scar of 5 cm to 10 cm on the neck, which is also difficult for many patients to accept. The ultrasound-guided microwave ablation of percutaneous thyroid nodules carried out in Zhabei District Central Hospital has the advantages of minimally invasive, accurate positioning, no scarring, no impact on thyroid function, less trauma to patients, shorter treatment time, faster recovery, fewer various complications, and exact efficacy. Under the leadership of Prof. Zhang Jianquan, we have formulated strict indications for thyroid tumor microwave ablation, scientific and reasonable treatment procedures, strict and controllable complication prevention measures, and a four-level comprehensive evaluation system, focusing on the preparation stage before treatment, with the core of preoperative preparation, intraoperative operation and postoperative evaluation, and paying attention to the individual characteristics of each patient. We have established innovative safety measures such as “liquid isolation method” and “thermal blocking blood flow coarse needle biopsy method” to put the safety of patients’ lives in the highest position. The minimally invasive treatment has truly brought into play the advantages of minimally invasive treatment and won the satisfaction and praise of patients.
