The parents thought that the child should not be different from other children when she went to kindergarten, so she consulted our hospital. After admission, she had the extra finger surgically removed, and the appearance of her hand was improved. Initial consultation The child is a female, 3 years old. Both hands were unfolded together, and it was found that the left thumb had polydactyly, with the polydactyly parallel to the radial side of the left thumb, in 2 joints, thinly developed, with normal nails, co-articulated with the thumb at the metacarpophalangeal joints, whereas the right hand was normal. The asymmetrical appearance of both hands is indeed aesthetic. In addition, the polydactyly can hardly be dominated to move independently and can only move along with the normal thumb. On the other hand, the thumb interphalangeal joints and metacarpophalangeal joints were movable. The purpose of this case was also clear to the parents, that is, to surgically remove the polydactyly, so the child was admitted to the hospital. After the child was hospitalized, the child was advised to undergo a left hand orthopantomogram in order to have objective imaging guidance during surgery. After the necessary blood tests were done and the results were basically normal, surgery was scheduled for the third day. The surgery went smoothly. After removing the extra finger, the normal thumb joint capsule was repaired to ensure its stability, the skin flap was trimmed to make it the right size, and finally, the skin was sutured intradermally. After the operation, the left hand was examined again with orthopantomogram, which contrasted with the preoperative period. Third, the treatment effect The child had wound pain on the day after surgery, and was given a child dose of diclofenac sodium enteric tablet for pain relief, and the symptoms were relieved; the pain symptoms were significantly reduced on the 2nd day compared with the day of surgery. At that time, considering the little girl, do not want to leave scars after surgery, and finally given cosmetic suture, after surgery the child’s surgical incision at only a thin line, to avoid the ordinary suture caused by the centipede leg appearance, affecting the aesthetic. Both hands were spread out together, and the shape of both hands was symmetrical and much better looking. After the operation, the child was allowed to move his left thumb, and it moved normally without any instability. The result of the operation was satisfactory, and after 10 days of hospitalization, the child was discharged with no redness, swelling, or oozing from the incision, and no signs of infection. Precautions We are glad that the child’s polydactyly was treated at an early stage. Although it was an intradermal suture, it was necessary to come to the hospital for wound care for 2-3 days after discharge and to keep the wound dry for 1 month. It is worth noting that one should avoid purchasing a dressing change kit to change the dressing by oneself, and should go to the hospital to be handled by a professional doctor to avoid causing infection. It takes at least 3 weeks for the joint capsule of the thumb to heal, so don’t move the left thumb frequently within 3 weeks to avoid the joint capsule is not firmly healed, which will affect the stability of the thumb in the later stage. Personal perception Polydactyly is not uncommon in the clinic, some of them grow on one side of the hand, especially parallel to the radial side of the thumb, because of the appearance of the opposite hand is not the same, and most of them do not have a function, it is generally recommended to surgical excision.
