Is amylase not high enough for pancreatitis?

Amylase is indeed a gold standard for the diagnosis of pancreatitis, but it is not unique and has some specificity. This is because in the early stages of pancreatitis, especially within 24 hours, blood amylase increases significantly and starts to decrease after 24 hours. If the onset of the disease is more than 1-2 days, it is possible that the blood amylase is not high, but it does not exclude the diagnosis of pancreatitis. We also have to combine clinical physical examination and other instrumental examinations, such as enhanced CT of the upper abdomen, which can clarify whether there is leakage around the pancreas. Even if amylase is not high, acute pancreatitis can be diagnosed if there is significant peripancreatic exudation. And amylase is not only the blood amylase, but also other criteria such as urine amylase is a diagnostic criterion for pancreatitis. Especially after a week of pancreatitis onset, the urinary amylase index level will increase significantly, which is also a reference indicator for the diagnosis of pancreatitis.
