Hemiplegic spasticity, also known as upper motor neuron paresis, is named after the increased muscle tone of the paralyzed limb. It is an important cause of stroke disability and has become the focus of stroke rehabilitation research. Modern medicine considers spasticity as an intermittent or continuous involuntary contraction of a muscle or muscle group, which is part of the upper motor nerve injury syndrome in central paralysis and is most severe during the spastic phase. Currently, rehabilitation medicine classifies the treatment of post-stroke hemiplegia into four phases: acute phase, spastic phase, relative recovery phase and sequelae phase, according to the clinical course. The spastic phase of hemiplegia is about 4 weeks after the stroke, which marks the end of the “shock period” of the spinal cord after the central hemiplegia, and the lower spinal cord centers have regained their control of motor ability. The lower level of primitive reflex activity occurs. The main manifestations are joint responses, co-movement, abnormally high muscle tone, and even spasticity. In some patients, even if they do not show spasticity and increased muscle tone at rest, they may show joint reactions and co-movement when they are engaged in active activities. This can directly affect the patient’s ability to take care of himself or herself in daily life, and even affect his or her quality of life. We believe that the prevention and treatment of hemiplegic spasticity should be carried out throughout the treatment of stroke disease. Early prevention of stroke disease and early and correct treatment after the onset of stroke are important factors to reduce the appearance of spasticity and prevent the disability of stroke, and Chinese medicine and comprehensive rehabilitation treatment of hemiplegic spasticity have shown their unique advantages. Traditional Chinese medicine and comprehensive rehabilitation have shown their unique advantages in treating hemiplegic spasticity. For example, the “Spiritual Pivot: Evil Guest” points out: “Evil gas and evil blood, solid can not stay, stay, then hurt the tendons and bones, organs can not flex and extend, so the contracture.” The “Difficult Classic – 29 Difficult” says: “Yin stilts for disease, Yang slow and Yin urgent; Yang stilts for disease, Yin slow and Yang urgent.” The evidence of the treatment of the Huixin “cloud:” “the disease has a lifelong not healed …… or flexion and can not stretch, or stretch and can not flex, in the hands and feet to make it. To wind injury liver, liver main tendons, tendons for it.” Jing Yue Quan Shu” also cloud: “where the yin deficiency blood less generation, can not support the camp tendons, resulting in contractures stiff servant, are this evidence.” It is believed that the spasticity of hemiplegia after stroke is either due to the imbalance of yin and yang, or due to the deficiency of yin and evil. Treatment is either based on the theory of yin and yang and emphasizes “dipping yin and replenishing yang”, or focuses on “replenishing deficiency and dipping actuality” from deficiency and actuality. For example, the “Acupuncture and Moxibustion A B Jing” said: “partial withering, hand blending small tendon urgency, Daling main. At present, the consensus is that post-stroke limb spasm is based on liver and kidney yin deficiency, and as the disease progresses, the limb constriction on the hemiplegic side due to the loss of tendon support is becoming the focus of conflict. Treatment is either based on the meridians of Shaoyang, Sun, Governor, Yang Wei and Yin Wei, or on the organs of the liver, kidney and spleen and stomach. Based on the experience of our predecessors and through a large number of clinical studies, we have summarized the basic pathogenesis and treatment rules of the hemiplegic spasticity state of stroke. Yang qi is the basic pathogenic mechanism of stroke hemiplegia and spasticity with blood stasis. Yang qi is the original driving force of the functional activities of human organs and bowels. As stated in “Suwen – The Book of Anger and the General Theory of Heaven”, “Yang Qi is like the sky and the sun, if you lose it, you will lose your life and not manifest”. It points out the importance of Yang Qi in the functional activities of human organs. It is also clearly pointed out that “Yang Qi is the essence that nourishes the spirit, and the softness that nourishes the tendons”. It indicates that the human spirit is warmed by Yang Qi in order to think quickly and energetically; the brain is warmed by Yang Qi in order to have sharp senses and smart ears; the tendons are warmed by Yang Qi in order to have gentle and free movement of the limbs; the veins are warmed by Yang Qi in order to have smooth flow of Qi and blood, suggesting that “Yang Qi deficiency” is the root cause of the spasm of tendons and veins. The “Nei Jing” also believes that “all spasms Xiang strong, all belong to the wet”, “because of the wet, the first as wrapped, wet heat does not hustle, large tendons short, …… short for detention”, ” evil evil blood …… organs can not flex and stretch, so detained also.” And so on, suggesting that dampness and blood stasis is the causative factor of tendon spasm. The wet is Yin, its nature is heavy and turbid, easy to hurt Yang, the so-called “wet Sheng is Yang micro also”, if the two get together, Yang more deficient, wet more heavy. Paraplegia spasticity occurs because of the stroke for a long time, excessive exercise, excessive application of pacifying liver and Yang, blood circulation and blood stasis products, injury to Yang and depletion of Qi, one of the Yang deficiency can not warm the tendons and veins, tendons lose warmth; the second is the Yang deficiency, Yang does not transform Qi, the fluid stops blood blockage, breeding dampness and phlegm, due to phlegm to stasis, phlegm and stasis, congestion of veins and veins, tendons lose tenderness, spasticity. Accordingly, we propose that “Yang Qi deficiency, dampness blocking blood stasis” is the basic pathogenesis of hemiplegic spasticity. Since Yang Qi deficiency plays an important role in the pathogenesis of spastic hemiplegia, Yang Qi deficiency is the root cause of hemiplegic spasticity, and dampness and blood stasis are the symptoms of hemiplegic spasticity, and Yang Qi deficiency and dampness and blood stasis are always present in the course of the disease. Therefore, we believe that the basic treatment for hemiplegic spasticity is to warm Yang, benefit Qi and eliminate dampness and blood stasis. To warm Yang and benefit Qi, dispel dampness and eliminate stasis, warm up the meridians and de-stasis the machine; invigorate the earth energy, strengthen the spleen and help transport and eliminate the source of dampness; supplement the source of the latter to benefit biochemistry, nourish the blood and soften the tendons of the body, treat with the evidence, the machine garden method is alive. In the treatment process, we should always pay attention to the protection of Yang qi, obey the Yang qi like warm but not wet, like moist but not hot characteristics, seize the tendons and veins like moist like soft this link, identify the evidence and treatment, targeted. In this way, the tendons and veins can be suppressed from the rigid quality to the soft body, in line with its striped and smooth nature. Based on our understanding of the etiology and pathogenesis of hemiplegic spasticity, we have summarized the “Xiangbei needle” and “Si Fa Si Fang” based on our clinical experience over the years, using acupuncture and medicine, treating the outside with Xiangbei needle and regulating the inside with Si Fa Si Fang. The four methods and the four directions are used to regulate the internal effects.
