Habitual subluxation of the atlantoaxial spine was first reported by CONER in 1907. It was defined as a habitual rotational subluxation by Wortzman and Dewar in 1968. Some authors have proposed the treatment of habitual rotational subluxation by surgery or posterior fusion. Other authors have suggested a conservative treatment of habitual subluxation of the atlantoaxial spine by manipulation followed by branching to maintain the repositioned position and subsequent clinical observation. Based on my own experience, I believe that a conservative treatment approach is possible and that good repositioning can be achieved in most children. In addition, the child needs to be carefully monitored for other complications, such as infection of the pharyngeal wall or cranio-occipital deformity.
