How can forced positions be prevented?

  Position is divided into active position and forced position. Clinically, forced position is the most common position, which means that the patient is forced to maintain a position because of the disease. The common forced positions are decubitus position, concave position, semi-sitting position, telescope position, lateral position, head-high-foot-low position and head-low-foot-high position.  Treatment: 1. The decubitus position is commonly used in postoperative patients undergoing intracranial anesthesia to prevent headache due to reduced intracranial pressure.  2, concave position is common in patients in shock, the purpose is to increase the amount of return blood, and thus increase cardiac output to reduce the risk of shock.  3, semi-sitting position is common in patients with abdominal inflammation or abdominal surgery, the purpose of taking this position is to reduce tension at the wound and relieve pain.  Prevention: 1, massage: can promote blood and lymphatic reflux, prevent or reduce swelling, the affected limb is also a sensory stimulation, conducive to recovery. Massage should be gentle, slow, rhythmic, moderate depth of action, do not use compulsory techniques; muscle groups with high muscle tone (such as upper limb flexors) with the nature of soothing pushing, so that they relax, while those with low muscle tone, such as upper limb extensors, massage and kneading, massage can be used with the meridian points to increase the therapeutic effect.  2.Passive movement: If the patient is still unable to start active activities after several days of coma or other reasons (such as total paralysis, serious comorbidities), passive movement of the affected limb joints should be done twice a day or more until the active movement is restored. The order of activities should be gradual and slow, from large joints to small joints, from small to large amplitude to stretch the contracted muscles, tendons and peri-articular tissues. In addition, the passive movement can be alternated or cooperated with massage, and the patient is encouraged to use the healthy limbs for passive movement appropriately.
