Be careful how to treat painless meatus hematuria

  We have recently encountered several consecutive patients with painless sarcoid hematuria, all with urological tumors. The longest period of discovery of hematuria in these patients was 3 years and the shortest was 1 month. Ask the patient who has a three-year history of hematuria, he started to have hematuria 2-3 times a year, “no pain, no itching, just take some anti-inflammatory medicine”, and did not care and did not go to the hospital for examination, but this year, the number of hematuria increased significantly and there was a blood clot blocking the bladder, unable to urinate, which led to a hospital examination, and the diagnosis was bladder tumor with hydronephrosis and Lymph node metastasis, which has lost the chance of surgery, which is very regrettable. Therefore, we are reminded that we must pay attention to painless carnal hematuria.  The urine of healthy people usually contains no or occasional traces of red blood cells. If certain diseases occur, it will cause an abnormal increase of red blood cells in the urine, which is clinically called hematuria. When there is a lot of blood, the urine becomes blood-red, which is called “hematuria of the naked eye”, and the cause should be promptly identified by a doctor.  Most hematuria is related to diseases of the genitourinary system, such as urinary tract infections, stones, prostate disease, tumors, and trauma. Most patients with hematuria are often accompanied by other symptoms, for example, hematuria caused by urinary system infections such as acute cystitis and urethritis is often accompanied by bladder irritation symptoms such as urinary frequency, urinary urgency and pain; hematuria caused by urinary stones is usually accompanied by acute lumbar colic, etc.  A small number of patients suddenly appear with no obvious cause, but no pain and other manifestations, clinically known as “painless carnal hematuria,” painless carnal hematuria is mostly seen in elderly patients, and is usually regarded as an important warning sign of urinary tumor, especially to be paid attention to. Older people are a high-risk group for tumors, and the most common urological tumors include bladder cancer, kidney cancer, ureteral cancer, prostate cancer and so on. Among them, bladder cancer is the most common and often manifests as flesh-eye hematuria throughout urination.  Of course, not all painless hematuria is caused by tumors, patients do not need to panic, they need to analyze the causes with the help of doctors, for example, painless hematuria can occur in elderly men with prostatic hyperplasia, but it is often accompanied by symptoms of lower urinary tract obstruction such as poor urination and nighttime; painless hematuria can also occur when the systemic coagulation function is abnormal; young people can have transient hematuria after strenuous exercise. Painless hematuria will disappear after a period of rest, which is called sports hematuria and is a normal phenomenon.  It is not terrible to have painless hematuria, even if it is urological tumor.
