Jaundice caused by a tumor blocking the bile ducts so that bile excretion is obstructed is called malignant obstructive jaundice. Usually doctors are used to divide into high and low obstruction, there is no very clear medical boundary between high and low obstruction. The usual demarcation is at the midpoint of the entire extrahepatic bile duct, which, under normal circumstances, is often near the opening of the bile cystic duct, but this is not absolute because of multiple variations in the opening of the bile cystic duct. There are four major types of clinically common malignant infarcts: 1. Hepatoportal bile duct cancer: high obstruction, often involving the intrahepatic bile ducts, only a small number of people are suitable for surgical treatment, interventional treatment (drainage with internal stent drainage) is ideal for inoperable cases, which can fully improve jaundice, followed by local tumor treatment and systemic treatment, the patient’s survival can be well prolonged and the quality of life is high . 2.peri-pot belly cancer: low obstruction, which is not a disease, usually considered to include pancreatic cancer, pot belly cancer, duodenal cancer and common bile duct cancer, although there are many types of diseases, but in fact the scope involved is not large, some patients choose surgery, some patients choose gastrointestinal endoscopic treatment, and some patients choose interventional drainage treatment. I personally advocate that surgery should be chosen if the tumor can be completely removed by surgery. Unfortunately, some doctors do open drainage instead of removing the tumor, which is a treatment I do not agree with. As for the choice of endoscopic treatment or percutaneous puncture is a controversial topic, patients can choose according to their own condition. 3.Hepatocellular carcinoma bile duct cancer embolism: it is very tricky, I have talked about it before. 4.Metastatic cancer (or lymph nodes) compressing the bile duct: most common in cases of gastric cancer (12 groups of lymph nodes) and colon cancer, interventional treatment is preferred. Drainage and stenting treatment itself is to improve the pathological state as physiological state, not to treat the disease itself, so don’t ignore the treatment of tumor after reducing yellow, you can choose ablation, radiotherapy, chemotherapy and other forms of treatment, I personally tend to mild treatment, because after all medical treatment is for “people”, not “disease”. “Therefore, the ultimate goal of medical treatment is to enable people to live in a dignified and healthy way.
